Job summary

An exciting opportunity has become available for a passionate and enthusiastic nurse to join the Heart Failure team at North Bristol NHS Trust. This team will deliver safe, effective, and timely clinical intervention to adult inpatients who are acutely unwell or have a new diagnosis of heart failure covering both inpatient and outpatient spells. This nurse led team will act with a high degree of autonomy to support high quality care for acutely unwell patients with heart failure.Practitioners in this team will respond to acute deterioration at an early stage and provide expert knowledge and understanding of patients in acute and chronic heart failure. They will provide an advanced clinical assessment and initiate investigation and clinical intervention to avert further deterioration. They will work in close collaboration with the parent specialty and ward teams to deliver high quality care and will support education of ward staff across the multi-disciplinary team.

Main duties of the job

The enhanced practitioners will be supported in developing as autonomous practitioners. They will assess, plan, implement and evaluate individual treatment interventions within the heart failure service and will work in close collaboration with the senior enhanced practitioners in the team. They will already have experience of cardiology more specifically heart failure and will thus bring a range of specialist skills and knowledge. They will continue to develop these skills in this role.
