Job summary

We have an exciting opportunity for an experienced cardiac nurse to join the NBT cardiac rehabilitation team. We are a fully certified service which meets the needs of over 600 patients annually who are recovering from a recent cardiac event. We can demonstrate excellent outcomes for our patients. A cardiac rehab enhanced nurse practitioner will be the patient's first contact after leaving hospital, they will support the patient in making the best possible recovery and equip them with what they need to remain healthy long-term. They will work alongside the patient and the MDT to address cardiovascular risk factors, provide education about heart disease, advice on symptom management, and exercise.

Main duties of the job

o To review patients on the ward to provide information for discharge, and discuss attendance at Cardiac Rehabilitation clinic and programme.

o To assess patients in outpatient nurse-led clinics which includes both physical and psychosocial needs. The assessment will involve giving essential post discharge advice to the patient, and where possible, family members.

o To run telephone outpatient clinics.

o To manage a caseload of patients and prioritise workload.

o To coordinate and lead an outpatient programme of 8 sessions which includes a cardiovascular exercise and health education.

o To maintain accurate patient records in accordance with the data protection act.

o To liaise with multi-disciplinary team members including GP's, Clinical Psychologists, Dieticians and Pharmacists.

o To keep up to date with advances in Cardiology/Cardiac Rehab and research pertaining to risk factor/ lifestyle advice as part of evidence based practice and continuing professional development.

o To work within national guidelines including the most recent NICE, British Association for Cardiovascular Prevention and Rehabilitation and the Association of Chartered Physiotherapists in Cardiac Rehabilitation guidelines.

o To support an effective referral process and consistency of delivery of information by providing teaching for both Primary and Secondary Care staff.

Band 6 cardiac rehabilitation enhanced nurse practitioners are expert clinicians, having completed a minimum of 4 years post-registration including a minimum of 2 years in cardiac nursing and possessing at least one relevant post-registration qualification. The post holder will act as an autonomous practitioner performing detailed clinical assessment and leading in the care of all patients in their caseload in collaboration with secondary care clinicians, community providers and the wider MDT. They will supervise and train band 3 and band 4 colleagues as well as students and will provide specialist advice to the rest of the MDT.

The post-holder will work across Southmead Hospital, Cossham Hospital and within community leisure centres across North Bristol.

The post holder will be responsible for implementing and delivering a safe and comprehensive cardiac rehabilitation program along with other members of the MDT. They will manage and participate in group exercise programmes for cardiac patients, and will provide exercise advice along with advice for managing cardiovascular risk factors.
