Job summary
An exciting opportunity has arisen for a Band 4 Nurse Practitioner (Assistant Practitioner/Nursing Associate) to join our Transitional care team. Our team comprises of midwifes, nursery nurses and healthcare support workers to provide care for up to 10 babies nursed with their mothers on a dedicated Transitional Care ward.
The role is integral to keeping mothers and babies together, supporting resident mothers as primary care givers for their babies with care requirements in excess of normal newborn care. Transitional care prevents admissions to the neonatal unit whilst providing additional support for small/preterm babies. As a successful candidate you will provide the highest standard of assessment, planning, implementation and evaluation of individualised care to babies being cared for on Transitional care.
Main duties of the job
- To promote a friendly and supportive atmosphere for parents and relatives in which they can learn to care for their babies
- To utilise a variety of strategies to communicate effectively, with the babies' family/carers, the multidisciplinary team, other departments within the the Trust and agencies outside the organisation.
- Perform observations on babies as required
- Providing breast feeding support
- Liaising with the neonatal team regarding transfers of babies to/from the unit or transitional care ward.
- Ensure parents/carers are updated on their baby's progress, being sensitive to their needs for courtesy, dignity and privacy, and always ensuring a friendly environment.
- To be and advocate for the infant and to promote good liaisons and effective communication with all members of the multidisciplinary team.
- To work closely with the midwife as registered supervision

Essential
- Higher Apprenticeship in Health Level 5 or Foundation Degree in Health and Social Care Practice or Higher Professional Diploma in Health and Wellbeing or a Recognised equivalent course (e.g. City and Guilds Level 4).
- Numeracy and Literacy Level 2 or equivalent
- Evidence of full time education achievements
- IT skills, basic word, Email, keyboard skills, data collection
- Ability and willingness to undertake appropriate training
- Evidence of commitment to continuing professional development
