Job summary

We are seeking enthusiastic Pharmacists to join our diverse Pharmacy team providing pharmaceutical care to a large Teaching hospital.

At North Bristol NHS Trust we are looking to recruit into our team of 20 rotational band 6 pharmacists. As a band 6 pharmacist you will rotate every 3 months through a variety of areas of general and tertiary specialties including surgery, respiratory, cardiology, medical admissions, neurology, stroke, mental health, medicines information and technical services.

We recognise the value of investing in our team and are proud to be able to provide the option of undertaking a funded 2 year Postgraduate Diploma in Clinical Pharmacy Practice plus the opportunity to study on an independent prescribing course, in a supportive environment with training opportunities, to build your foundation skills and develop your expertise.

Our pharmacists are vital members of the multidisciplinary team and you will be providing clinical support, medicines reconciliation and patient counselling services to patients throughout the Trust.

Applications are welcome from registered pharmacists and trainee pharmacists due to take the General Pharmaceutical Council exam in June 2024.

Main duties of the job

  • Providing patient centred pharmaceutical care ensuring patient safety issues are prioritised, challenged, and resolved in a timely and appropriate way.
  • To support the use of medicines across the Trust to achieve best outcomes for patients in accordance with best practice.
  • To prevent and reduce medicines related harm.

Our rotational posts allow Pharmacists to develop and gain experience in a number of clinical specialities including Neurology, Respiratory, Cardiology, Admissions, Surgery, Women's health, Mental Health, Technical Services, Quality Assurance and Medicines Information.

We are a large pharmacy department with over 80 pharmacists including many Specialist posts. The clinical ward pharmacy service is well established and provided by clinical pharmacists and medicine management technicians. The pharmacy department is located in our award winning Brunel building, which is a state-of-the-art modern hospital that opened in 2014. Witha large staff only restaurant, caf and rooftop terrace seating area with views across the city, the Brunel building offers staff the opportunity to enjoy comfortable, light filled areas to work and rest.   • Participate in the dispensary, ward, QA, weekend, late shift and on-call rotas contributing to service delivery. Adhere to all established procedures and complete all necessary records.
  • Clinically screen prescriptions, and discharge summaries at ward and dispensary level, to ensure the safe, efficient, evidence-based and rational use of medicines in line with the Trust Formulary and departmental policies so that risk is minimised, and patient flow optimised.
  • Record all major interventions made.
  • Medicines Information deal with routine enquiries that arise on the ward and in the dispensary. Refer on complex queries to the medicine information team.
  • Provide advice and information to patients/carers to ensure compliance with treatment.
  • Ensure that a full medicine reconciliation is undertaken for each patient using the appropriate sources (GP, community pharmacist, patient/carer, clinic notes, nursing home).
  • Dispensary - provide professional supervision of the dispensary area on a rotational basis. Provide final accuracy check of dispensed items.
  • Check issues of controlled Drugs and participate in departmental audits of CDs.
  • Handout oral chemotherapy prescriptions and counsel patients as prescribed on chemocare.
  • Provides an input into Quality Assurance (QA) and quality management systems for the Pharmacy department and Trust under the supervision of the QA team.
