Job summary
This is an exciting, new opportunity for a passionate and driven senior practitioner to take up a newly created role of the Advanced MS Pracitioner (AMSP) within the MS service at North Bristol NHS Trust. The AMSP will hold a professional registration with either the NMC or HCPC and will work as an integral member of the multi-disciplinary MS service.
The post holder will provide strong leadership and effective consultancy for patients with advanced multiple sclerosis both internally and externally to the Trust and wider multi-disciplinary teams.
Coming from a strong background of advanced clinical practice and extensive experience working within neurology, the role of the Advanced MS Practitioner will guide care for people with advanced MS who are facing multiple, complex problems with the focus on maintaining and improving function and independence wherever possible.
This will be achieved by concentrating on ensuring services for people with advanced MS are optimised and will help to transform MS services for people with advanced MS.
The post holder will be given significant support to design and structure this new service around the needs of our patient cohort and will be equally supported via the MS Trust who also continue to support and offer ongoing training for this role throughout the UK.

In brief, this post will:
- Plan care in partnership with the person with advanced MS, and their carer, actively facilitating their choice and involvement in decision making.
- Coordinates programmes of care with other health and social care professionals
- Independently conduct neurological examinations and objective measures of patient function
- Competently review and interpret data generated through these assessments and revises the care plan in conjunction with the person with advanced MS, their carer, and other appropriate members of the team accordingly.
- Apply high-level assessment and consultation skills
- Ensure the needs of people with advanced MS who are admitted to hospital as an emergency are met effectively, reducing length of stay and risk of readmission
- Establishes systems to identify people with advanced MS who may be at increased risk of admission and develops and implements personalised action/care plans to minimise this risk.
- Facilitates access to end-of-life care and support as required.
- Identifies people with advanced MS who would benefit from access to specialist equipment such as seating, profiling beds, environmental equipment etc.
- Supports and advises colleagues who are caring for people with advanced MS to help them to deliver high quality, cost effective, specialist care, which optimises outcomes for people with advanced MS and their families.

Key Process Indicators (KPIs) will include:

- Improvement in outcomes for patients with MS and their families
- Improved equipment provision or help to meet other specialist needs
- Instigation of advanced directives and discussion of end of life care with patients, close family and clinical agencies
- Increased support and training for other HCPs working with patients with advanced MS
To enable the delivery of the KPIs the post-holder will use their leadership skills and specialist MS knowledge to establish strong links with other key colleagues, both internally and externally including Clinical Commissioning Groups (CCGs) and Community Services across the region.
