Job summary
This is a new role within the Sarcoma team, supporting patients throughout each stage of their pathway. We are recruiting for a fixed period of 12 months for a total of 56 hours per week. We therefore welcome applications from candidates interested in part or full time working to fill these hours.
Under the guidance and supervision of the Clinical Nurse Specialist (CNS) team the role will facilitate and support:

They will be part of the wider Sarcoma team working closely with Sarcoma clinicians, Sarcoma clinical nurse specialists, Patient Pathway coordinators, diagnostic teams, Cancer Services, psychology, and allied health care professionals.
They will be supported to develop knowledge of sarcoma, the treatment of sarcoma and the resources available to patients and carers.Support the clinical team to ensure the speciality can deliver timely, safe and effective services.
Provide support and information to patients throughout the sarcoma pathway, produce data and governance leadership (in line with National Sarcoma Service Specification, national cancer dataset (COSD) and national cancer waiting time targets) for the regional Bristol Sarcoma Service.

The post-holder will be responsible for coordinating and supporting the work of the sarcoma specialist nurse service and providing a point of contact for patients and professionals.
Provide support to enable people to navigate the health and social care system and make choices that are best for their cancer and their life.
The information and nature of all communication required is sensitive due to the nature of cancer. Communication in this context requires a high degree of empathy, understanding, diplomacy, honesty and integrity.
This role will undertake the tracking and reporting for all soft tissue sarcoma patients across our wide regional catchment area:
To ensure the management of patients is efficient and effective, taking into account the individual needs of the patients and the requirements of the service. To contribute to the achievement of the National Cancer Waiting Times targets, National Audits and Cancer Registry.
To play a pivotal role in supporting the sarcoma Multi-disciplinary Team (MDT) in co-ordinating services to deliver the specified outcomes and targets of the Cancer Plan.
To have an active role in redesigning pathways and service improvement, particularly in the analysis of existing pathways to enable a continuous programme of development, including streamlining MDTs and the implementation of whole gene sequencing into the soft tissue sarcoma pathway.

- Develop an in depth knowledge and understanding of the pathway for sarcoma patients; including an understanding of medical terminology relating to Sarcoma and Cancer.
- Where necessary to liaise with other Clinicians and MDT Coordinators/administrators outside of the Trust.
- To support clinical teams and help identify and implement pathway changes that will improve compliance with national waiting times standards for cancer.
- Works closely with the clinical nurse specialists to track patients throughout their cancer pathway and flag up any delays in the cancer patients journey. Ensure all outcomes of the MDT meetings, tests, investigations, and referrals are recorded and acted upon.
- To be proficient in the use of multiple hospital IT systems, including CRIS, CareFlow, ICE and Somerset Cancer Register.
- Undertake holistic needs assessment and the development of an personalise care and support plans.
- Implement, monitor and review the care plan with the patient and carer and modify as appropriate.
