Job summary

A fantastic opportunity has recently arisen within Core Clinical Services as we are advertising for a Patient affairs officer working 15 hours a week to join our busy Patient Affairs team.

The post holder is responsible for providing expeditious and compassionate bereavement service to families and next of kin following the death of a loved one and undertaking all necessary administration tasks required to provide a complete bereavement service for all adult deaths in the trust.

The post holder will work closely with multiple teams including the Mortuary team, The Chaplaincy team, palliative care team, and the Medical Examiner service in their work to ensure that all necessary administrative tasks are carried out in a timely and efficient way.

You will be part of a small but busy team working to ensure the Trust provides a high quality and complete bereavement administration service.

Main duties of the job

The Patient Affairs Officer is responsible for supporting the bereaved following the death of a loved one by providing all necessary information in a caring and compassionate manner and ensuring all administrative processes are completed in a timely manner.

To play a central role within the after death care service and to work in to liaise with colleagues, staff in other departments, GPs, bereaved families and Next of Kin. To work well as part of a team whilst being able to use their own initiative to ensure effective high quality service is provided.

To be the first point of contact within the after death care service therefore the ability to problem solve is vital for this position.

Job responsibilities

- To be the first point of contact for the after death care service.

- Communicate sensitive or contentious information in relation to bereavement where there may be barriers to understanding.

- Receiving and logging daily notification of adult deaths.

- Provide notification to primary care regarding deceased individuals.

- Raise and Maintain PAS and spreadsheet records for deceased patients.

- Support the Medical Examiner Office in planning and organising the Medical Examiner processes through requesting and preparing medical records.

- Receiving, recording, storing, and issuing of patient property.

- Identify, contact, and arrange time for appropriate doctor required to complete Medical Certificate of Cause of Death (MCCD).

- Pass information to MEO for contact with family, next of kin and doctors.

- Support QAPs in referring cases to coroners Office when instructed by MEOs to do so.

- Contact Family / Next of kin to ascertain and discuss proposed funeral arrangements and contacting the registrars office.

- Maintain accurate records for all information, decisions and records on PAS and other Datasets used within the service.

- Ensuring timely and accurate completion of certification (MCCD) following the death of a patient.

- Ensuring that all contact with next of kin, other family members or those registering the death remains professional, sensitive, and compassionate always.

- Accurately recording, storing, and arranging the collection of patient property.

- Meeting bereaved family / next of kin or others attending to collect property.

- Issuing and explaining the Trust bereavement booklets.

- Providing information about the trust complaints service when appropriate or directing people to the information contained within the bereavement booklet.

- Arrange for further discussion with ME, doctor, or consultant if family or next of kin have an ongoing concern

- Provide information and advise to staff regarding matters relating to the death of a patient and the provision of bereavement services.

- To Identify, contact, and arrange collection of doctors cheques.
