Job summary
NHS@Home (originally Hospital at Home service) is an exciting model of care that bridges the gap between traditionally hospital-based care to care in people's home, supported by joint working with system partners and the use of appropriate remote clinical monitoring tools.
We are seeking a skilled, highly motivated dynamic Nurse to support the delivery of patient care in the community to support the hospital based team to manage & organise care provisions.
You will be responsible for undertaking numerous community patient visits daily to provide a variety of clinical interventions. The role is based at Southmead Hospital & you will be employed by NBT & entitled to NBT benefits. You will be required to work across Bristol, North Somerset and South Gloucestershire. You will also be required to support the hospital based team organising care provisions in collaboration with the wider multidisciplinary team, attending meetings with the support of senior staff, and supporting in -reaching, assessing in-patients for service suitability.
This is a diverse role that requires flexibility and the ability to work effectively under pressure. You will undertake lone and working in the community whilst having the constant support of the Senior Sister, also working with collaborative partners to ensure patient needs are efficiently and effectively managed providing high standards of patient care at all times.
This is a permanent post however secondment will be considered.

- The post holder will be designated patient community visits daily, to deliver patient care using a variety of clinical skills- IV therapies , venepuncture, cannulation, VAC wound therapy and drain care (not an exhaustive list)
- Ensure delivery of high quality, responsive, safe, efficient, and compassionate care.
- The post holder will work in collaboration with the NHS@Home team at Southmead hospital and BNSSG system partners at Sirona and UHB.
- The post holder will take personal responsibility and accountability for ensuring that Trust and local policies and procedures are known and followed and will ensure duties are carried out in accordance with the advisory guidelines and practice set out in the NMC code of Professional conduct.
- The post holder will be responsible for supporting hospital based patientassessment, care planning andprovision, maintenance and evaluation of patient care.
- The post holder will appropriately escalate patient deterioration or change in condition effectively and in a timely manner in line with NHS@Home procedure.
- The post holder willsometimes take responsibility for the supervision and guidance of other staff including health care assistants and students, providing a good learning environment.
