Job summary

The Memory Assessment Diagnostic Service provides a comprehensive, person-centred pathway for the timely identification, assessment, diagnosis, and early post-diagnostic support of individuals experiencing concerns about memory or cognitive function. The service aims to ensure that all patients referred with suspected cognitive impairment receive a high-quality, evidence-based assessment delivered by an integrated multidisciplinary team.

The team is made up of Consultant Psychiatrist, Clinical Psychology, OT, Nursing and Health Care Assistants who deliver the comprehensive diagnostic and treatment pathway.

The team prides itself on its collaboration with local Alzheimer's Society and Carer's Agencies, along with frailty services and Innovations in Dementia to ensure that all living with dementia receive holistic onward interventions, providing hope and enabling those receiving a diagnosis to live well with Dementia.

The role will involve delivering services throughout the diagnostic pathway, from triage to transfer to other services. The post holder must be able to demonstrate empathy and sensitivity to those using the service and their supporters throughout the pathway.

Main duties of the job

The specialist nurse:

completes initial clinical assessments, including cognitive screening, risk assessment, physical health checks (where relevant), and collateral history gathering. Supports diagnostic formulation within the MDT. The nurse is part of the referral screening - identifying those people for who dementia diagnosis is appropriate - having a thorough knowledge of co-morbidity, both physical and mental health, that can impact on cognition and making onward referrals to support the patient to receive the right care

Delivers education and advice to patients and carers regarding cognitive health and lifestyle. Includes specialist treatment recommendation in the case of non medical prescribers and offers advice and guidance to GP's where necessary.Provides post-diagnostic support, within agreed protocols and acts as a key contact for patients and carers throughout the pathway. The Band 6 Specialist nurse offers supervision to the band 5 nurses.
