Specialist Community Mental Health Nurse

These posts will be based in Bridlington with expectation to work into Driffield when service or leadership demand requires as we are one team across two bases. Due to the geographical nature of the post and how care is delivered it is essential that you can drive and have access to a car.

There are two part-time posts available, each for 30 hours per week. One role runs Monday-Thursday, 9am-5pm, and the other runs Tuesday-Friday, 9am-5pm.

Main duties of the job

As a key worker / care coordinator you will provide support and interventions to promote mental health recovery. Establishing therapeutic relationships with service users and their family / carers.

• Develop and deliver recovery focused care in collaboration with service users and their family /carers.
• Provide information and psycho education surrounding mental health conditions and medication.
• Uphold legal frameworks which underpin service delivery.
• Assess risks, the management of risks and effectiveness of interventions.
• Deliver recovery based and risk management interventions with complex clients.
• Signposting / referral to external services when required.
• Develop good working relationships with neighbouring teams to enable effective co-working and responsive transfers of care if necessary.
• Act as duty officer on a rota basis
• Administration and monitoring of medication.
• Supervise other members of the team and students.
• Leadership to support the team day to day, in meetings and with clinical decision making.
• Deputise for leadership when required.
