Job summary

An exciting opportunity has arisen for a Band 5 Mental Health or Learning Disabilities Nurse to join our Mental Health Crisis Intervention Team, and we also welcome applications from nurses in preceptorship or soon to qualify.

We are a 24/7 Crisis and Home-Based Treatment service supporting adults aged 18-64, along with their families and carers, across Hull and East Riding who are experiencing an acute mental health crisis. The team provides crisis assessments, home-based treatment, gatekeeping for inpatient admissions, and coordination of the Section 136 Suite/Mental Health Place of Safety at Miranda House. The work is varied, fast paced, and offers rich clinical experience.

We work closely with Yorkshire Ambulance Service, Humberside Police, primary and secondary care, and voluntary sector partners, as well as collaboratively with families and carers. We are seeking proactive, confident, and adaptable individuals with experience of supporting people with mental health difficulties and complex needs. As an active member of the multidisciplinary team, you will contribute to high-quality, person-centred care and the ongoing development of the service.

We are committed to supervision, learning, and professional development. You will receive regular managerial and clinical supervision through supportive forums to ensure you have the guidance and development needed for the role.

Main duties of the job

As part of the Crisis Resolution and Home Treatment Team, you will carry out timely mental health assessments, identify risks, and support individuals experiencing acute mental health crises. You will deliver short-term, intensive home-based interventions aimed at stabilising mental state, preventing hospital admission, and promoting recovery. This includes providing therapeutic support, administering medication, monitoring mental and physical health, and responding flexibly to rapidly changing needs.

You will work collaboratively with the wider multidisciplinary team, families, carers, and partner agencies to ensure coordinated, person-centred care. You will contribute to care planning, maintain accurate clinical documentation, and participate in daily reviews and handovers. The role involves regular home visits, rapid response to urgent referrals, and participation in a 24/7 rota including evenings, nights, and weekends.

You will engage in supervision, reflective practice, and ongoing professional development, while supporting service improvement and upholding trust values in all aspects of care.
