Job summary
An exciting opportunity has arisen to be part of Humber Teaching NHS Foundation Trust's Community Ageing Well Programme. We are currently recruiting on a part time bases at 15 hours per week .
The successful applicant will be appointed to support the integrated service delivery for the VirtualWard service and support into our wider ageing well service, VirtualWards allow patients to receive the care they need at home, including care homes, safely and conveniently. They also provide an opportunity to narrow the gap between demand and capacity for secondary care beds, by providing an alternative to admission and/or early discharge.
The model delivery will be within Scarborough, Whitby and Ryedale community, with a focus on admission avoidance and early supported discharge for people living with frailty in Scarborough. The service will operate 7 days a week, for a minimum period of 08:00-20.00. As a ongoing developing service, there may be a requirement to extend hours for administrative support beyond 5 days as the service develops.
Main duties of the job
- To provide effective and efficient administrative support to the North Yorkshire Virtual Ward and ageing well service within Humber Community Services.
- Provide clerical support to associated clinical staff.

- To provide and receive routine information orally, in writing or electronically to inform work colleagues, patients, clients, carers, the public/external contacts.
- To demonstrate a competent level of inter-personal skills when providing information of a non-clinical nature to staff, clients, carers, public/external agencies, maintaining the Trust standards of customer service.
- To have an understanding of security/health and safety processes and procedures, as required, relevant to the area of work
