Job summary

We have an exciting opportunity for a full time Forensic Practitioner post within the Adult Forensic Community Team on the Mental Health and Personality Disorder Pathway. The team is based at the Humber Centre for Forensic Psychiatry.

The Forensic Practitioner will support the delivery of specialist assessment, inreach and therapeutic intervention. These services are designed to support individuals who are transitioning from secure settings back into the community. The teams also provide a consultation and assessment service to support community mental health teams.

The post will be responsible for delivering safe, effective and evidence-based care. They will work as part of a multi-disciplinary team and deliver therapeutic intervention in the context of a trauma informed model of care.

The successful applicant will need to have the means to travel between various geographical distances.

Main duties of the job

The Forensic Practitioner will support the delivery of specialist assessment and therapeutic intervention with the community forensic teams.

Key Responsibilities include:

Clinical Responsibilities

Utilise specialist forensic skills in consultation, assessment, and intervention.

Deliver clinical care and promote evidence-based practice.

Assist in assessment, formulation and care planning.

Use critical thinking and clinical decision-making skills.

Handle distressing situations sensitively, including managing complex situations.

Ensure patient safety.

Multi-Disciplinary Working

Support therapeutic interventions within a multi-disciplinary team.

Provide clinical support and advice to colleagues.

Work autonomously and collaboratively within the team.

Engage with service users and carers.

Service Development

Participate in training and research projects.

Maintain high standards of documentation.

Manage resources.

Support managers with service development projects.
