Job summary
We are seeking a band 7 Respiratory Specialist Nurse to lead our Home Oxygen Service. The Home Oxygen Service sits within our wider community respiratory team which includes respiratory nursing, home oxygen, pulmonary rehabilitation, and respiratory outpatient physiotherapy. Candidates need to have a keen interest in respiratory nursing and established respiratory knowledge and skills.
The post involves providing support and education for respiratory patients discharged from hospital and leading within the Home Oxygen service in the community, providing leadership, training and staff support.
The post holder will lead the Home Oxygen Service with responsibility for the provision of a coordinated service to facilitate the integrated and proactive delivery of care to patients on home oxygen therapy and with a range of respiratory disease across Scarborough, Ryedale and Whitby areas.
Working pattern and base within the service delivery area can be discussed.
Main duties of the job
- To ensure complex patients requiring home oxygen therapy receive high quality, holistic, evidence-based, cost-effective clinical assessment, intervention & review, underpinned by national guidelines, evidenced based practice and current clinical guidelines.
- To work closely with other Band 7 respiratory colleagues to support and ensure the smooth running of the community respiratory services.
- To manage a clinical caseload, providing advice and support to patients using home oxygen, and maintaining overall responsibility for clinical caseload management.
- To continue to demonstrate advanced clinical skills in practice and provide support and advice re service development and the delivery of care in partnership with patients, relatives, staff, managers, & external agencies, across a number of teams/geographical areas
- To liaise with multi-professional caseload holders and case managers regarding patients on their caseloads
- To work with primary care to develop relationships and support general practice with home oxygen prescribing and queries and community respiratory advice.
- To liaise with the home oxygen provider, to ensure up to date equipment and service knowledge and be the patient advocate as required.
- Involvement with specialist professional respiratory networks
