An exciting opportunity has arisen for a senior Case Manager at the Humber and North Yorkshire Mental Health, Learning Disability and Autism Collaborative.
This role will involve directly managing a designated caseload of patients within a defined group.
The successful candidate will be:
- Registered Mental Health / Learning Disability Nurse or other registered health or social care professional
- Educated to master's level or equivalent level of experience of working at a senior level in specialist area
- Will have a working knowledge/experience of Case Management

Main duties of the job
As a Mental Health Case Manager, the post holder will work as part of a dynamic team in delivering an effective service; supporting managers and staff across Humber and North Yorkshire to ensure safe and effective care pathways are in place for service users of specialised mental health care settings within the NHS and Independent sectors.
This role will provide expert clinical support to the Quality Assurance and Improvement Lead and Commissioning Leads in relation to the defined population. This will include projected mapping of future service use, identification of users for future service provision, planning in terms of future service needs.
The role supports the business in driving transformational high-quality care as well as value for money in planning and quality assurance. The role is designed to build a combination of subject matter expertise and technical skills to develop a strong service delivery.
Improving quality and outcomes through clinical assurance of the care provided by the relevant specialised services.
Managing individual patient pathways to ensure high quality services and care planning outcomes are achieved
Enabling patient and public involvement
Operationally oversee and support the delivery of day-to-day activities and projects
Provide operational and clinical cover for other Case Managers within the HNY provider collaborative when needed.
