Job summary
An exciting opportunity has arisen for a complex case holder band 7 who has drive and is looking for a challenge to join the community services in the beautiful market town of Malton. The post holder will work in an integrated multidisciplinary team working closely with the district nursing teams, urgent care response team, social care and GP colleagues across traditional care boundaries in the integrated delivery of individualised, safe and effective, high-quality care.
The post holder will work to proactively identify, case manage, care and treat adults and older people who have varying health needs which includes frailty, complex and long-term care needs. Through this process the aim is to ensure patients stay at home or their normal place of residence, avoiding unnecessary hospital admissions and where patients have been hospitalised, to facilitate an early discharge. The post holder will be able to assess patients' needs, be able to quickly identify a deteriorating patient.
The post holder will work as an autonomous practitioner and will confidently working collaboratively with colleagues across a wide range of professions and organisations. V300 prescriber desirable but not essential as training will be given on the V300 course, experience of managing staff and a working knowledge of System 1 would be an advantage.
Main duties of the job
You will be the Senior Nurse or AHP for complex care and frailty for people in their own homes.
Co-ordinator between GPs, social care, acute hospitals, discharge planning and patient assessments as well as working with the district nurses and UCR team.
To be able to take on responsibility of the team when the clinical Band 7 lead is on Annual Leave.
