Job summary

Are you a band 5 or band 6 nurse with experience in older peoples mental health? We are looking for passionate, creative and ambitious nurses to join our new care home service working with those needing increased support living in a care setting.

We are seeking people with a drive towards service improvement and providing high quality care for those in care settings. You will be able to demonstrate experience in older peoples mental health care, managing risk, and providing positive person centred intervention.

Communication with people is a vital part of this role, we are looking for nurses who can provide education to care staff in a meaningful way that benefits those we are caring for.

Main duties of the job

The team will provide intervention for those living in care settings who require increased support as a result of their mental health needs, based on evidence based practice as part of an multi disciplinary team.

The service will cover Hull and East Riding care homes and staff will need to be able to travel to ensure we can offer a responsive service.

Our nursing staff will provide assessment, person centred care planning, clinical intervention, risk assessment and review whilst modelling high quality care and providing education.

The applicant will have advanced working knowledge of associated legislative frameworks including the Mental Capacity Act (2005).

A major part of the role will be engaging care settings and developing trusting , honest relationships to ensure those in care receive high standard mental health care. Nurses will take the lead and become a main contact for care settings within Hull and East riding to promote continuity.

Nursing staff will be part of a rotation between the care home service and Maister Lodge, our organic inpatient unit to ensure that skills and confidence is maintained and to further promote staff development.
