Job summary
The opportunity has arisen for a Band 5 Registered Learning Disability Nurse to work in the Learning Disability Intensive Support Team (LD-IST).
The LD-IST is a multi-disciplinary team offering therapeutic interventions to clients who may present with significant behaviours of concern and/or Mental illness. The team is underpinned by the Transforming Care Agenda and Building The Right Support. The team aims to ensure clients are supported in the community and avoid hospital admission. Where hospital admission is required the team support prompt and safe discharges.
The successful applicant will join a MDT of nurses, care coordinators, psychologists, Occupational Therapists, Speech and Language Therapists and nurses and healthcare assistants to provide evidence based, person centred care.
We welcome applications from newly qualified nurses as well as experienced nurses.
Main duties of the job
- Person centred care planning to support clients who experience significant behaviours of concern.
- Supporting and completing risk assessments
- Supervise and support Health cares assistants within the team
- Mentor students
- Work as a member of the multidisciplinary team
- Develop partnerships with other agencies to support individuals

- A broad understanding of relevant policy, legislation drivers and their application to clinical and service area.
Desirable
- Demonstrable experience of mentoring pre-registration students
- Evidence of current CPD/formal qualifications at an advanced level e.g. degree or post graduate level or equivalent knowledge/experiential learning
