Job summary
The London North West Healthcare NHS Trust consists of Northwick Park, Central Middlesex and Ealing hospitals.
The Complex Discharge Team are looking for an innovative and enthusiastic patient pathway Co-ordinator who will bring new ideas and share their excellent working practice within this busy, friendly and supportive team.
Integrated Discharge Team is committed to providing a 7 day service and this role may include participation in Weekend and Bank Holiday cover.
To succeed in this role, you will need excellent communication skills and extensive experience in managing complex discharges in the NHS.

Main duties of the job
Have the professional knowledge and leadership skills to take the lead in co-ordinating complex discharges to ensure together with ward senior staff that the Discharge Pathway is adhered to enabling a safe, smooth discharge.
Be responsible to optimise hospital bed utilisation and reduce the length of stay by ensuring the timely, safe transfer/discharge of patients to the most appropriate and agreed environment. This will include monitoring the transfer of patients to non-acute rehabilitation centres and facilitating delayed transfers of care.
Instigate and co-ordinate meetings with patient, family/carers and community agents and facilitate joint working partnership between Primary and Secondary Care.
Ensure that staff training and procedures are adhered to, promoting a patient centered care pathway to reduce delayed transfers of care with the Trust.
Be part of the review and implementation of the LNWH NHS Trust Discharge Policy within all departments, proposing and working towards service changes within multi-disciplinary teams or outside own specialist area.
