Job summary
The overall purpose of this post is to support the delivery of the End of Life Care Strategy within LNWH. The post holder will be involved in the delivery of nursing care for patients in the last phase of life with the level of knowledge, skill and competence beyond that of a traditional healthcare assistant. You will support and contribute to education and training of registered nurses, healthcare assistants and other members of the multi-disciplinary team (MDT), with regards to the specific needs of patients and their families/carers at the last phase of life.Working under indirect supervision, as part of the nursing team, you will have responsibility for delivering nursing care to patients within a delegated caseload.You will participate in multidisciplinary working and the development, implementation and evaluation of the delivery of health care services.
Main duties of the job
o under indirect supervision, as part of the nursing team, the post holder will have responsibility for delivering nursing care to patients within a defined nursing caseload.o Recognise when patients are likely to be dying, and share this concern urgently and effectively with other members of the teamo Contribute to care planning and reviewso Contribute to the discharge planning of patients.o Assess and respond to symptoms of dying (e.g. pain, agitation, nausea, respiratory secretions)o Assess and respond Working to the needs of the people close to the dying persono Assess and respond to the spiritual needs of patients and carerso Day to day involvement with patients and clinical teams in delivering and evaluating effective patient-centered care, with support from a registered nurse as requiredo Recognise and respond appropriately to urgent clinical situations, being aware of own competence and asking for assistance as required.
