Job summary

An opportunity has arisen within St Mark's Hospital Stoma and Pouch Care dept. We are looking for a highly motivated senior nurse with vast experience of colorectal and GI nursing to lead our team of clinical nurse specialists based at St Marks at Central Middlesex & Northwick Park hospitals.

The stoma & pouch care dept at St Mark's Hospital is one of the largest teams in the UK for stoma and fistula management. As a tertiary referral centre, we create significant number of new stomas and ileoanal pouches per annum. We also offer long-term support to community patients in the local boroughs.

This senior Nurse, Consultant position will be both managerially and clinically responsible for a team of clinical nurse specialists and will lead expert practice of nurse-led ileo-anal pouch, intestinal failure and stoma care services throughout the LNWH NHS Trust. The post holder will be fully responsible for a clinical workload acting as an independent practitioner and continually strive to improve patient focused services through the development of nurse-led clinics centered on national and international stoma care guidelines.

The post holder will lead in the development and delivery of multi-professional training programmes in the field of ilea-anal pouch, intestinal failure and stoma care driving expert practice through evidence-based practice, clinical audits and research to ensure maintenance of clinical excellence in conjunction with the St Mark's Academic Institute

Main duties of the job

The post holder will provide long term management and support to patients with a stoma or fistula and lead the stoma & pouch nurse specialists within the team. The post holder will be expected to work cross-site, Central Middlesex and Northwick Park Hospital sites.

The post holder will be expected to apply advanced physical assessment and clinical history taking skills to analyse and interpret highly complex medical and surgical information to formulate a plan of care for a patient with an ileo-anal pouch, stoma or intestinal failure within a nurse led clinic.

The post holder will be expected to communicate effectively with all members of the multi-disciplinary team within the Trust, regionally and nationally to achieve a coordinated specialist service for patients.

The post holder will be expected topublish and present research findings and audit within national and international forums, including peer reviewed journals and at nursing and medical conferences.

The post holder will fully independently carry out rigid pouchoscopys', digital and physical examination within the nurse-led clinic as a means of diagnosis and long-term surveillance of patients with an ileo-anal pouch.

The post holder will provide highly specialist nursing advice, support and counselling to patients and their families with regards to their ilea-anal pouch, fistula or stoma within the Trust facilitating a safe discharge into the community.

Job responsibilities

Provide long term management and support to patients with a stoma or fistula and lead the stoma & pouch nurse specialists within the team. The post holder will be expected to work cross-site, Central Middlesex and Northwick Park Hospital sites.

  • Demonstrate highly developed interpersonal skills to be able to communicate with all members of the multi professional healthcare team within the Trust as well as regionally and nationally in order to achieve a coordinated specialist service for patients. This will involve communicating specialist and highly complex or sensitive information verbally, formally written and electronically as appropriate.

  • Utilise expert communication skills to disseminate and simplify highly complex surgical information, using appropriate information aids as necessary, in order to facilitate the patients understanding and inform the patient of their surgical options so the patient can give informed consent. Discuss results of investigations and ongoing care.

  • Utilise advanced communication skills to identify patients at risk of clinical deterioration in medical condition from telephone communication. To receive,analyse and interpret highly complex medical and surgical information and to formulate a plan of care or to make appropriate referrals i.e referral to Consultant Surgeon. This will avoid unnecessary appointments in the Out Patient Department.
  • To deliver individualised care and psychological support to patients and families incorporating their diverse cultures, ethnic backgrounds and any disabilities they may have.

  • To ensure accurate records of patient care are maintained i.e. patient notes and referral letters and that relevant clinical information is transferred onto the trust database.

  • To be skilled at making presentations using multimedia tools at local meetings to all members of the multi professional team in order to share best practice.

  • To liaise with home delivery and product companies the services and equipment required by patients.

  • To publish and present research findings and auditS within national and international forums, including peer reviewed journals and at nursing and medical conferences.
