Surgical Escalation Lead (SEL)

Job summary

The Surgical division at the Northwick Park Site are looking for a Band 6 RGN to work as a Surgical Escalation lead. This is a full time permanent post and the post holder will work across 7 wards, ensuring that all escalations are completed to ensure that the patients pathway through the trust is expediated.

Main duties of the job

The post holder will have the nursing professional knowledge and leadership skills to take the lead in troubleshooting, communicating and assisting the ward discharge co-ordinators and lead nurses, Clinical Site Practitioners to ensure that all discharge delays are expediated quickly and efficiently.

They will be responsible for optimising hospital bed utilisation and help to reduce the length of stay by ensuring timely, safe transfer/discharge of patients to the most appropriate and agreed environment. This will include being the escalation lead for routine delays in a patient's pathway. any delays in booking EEG, CT scans, X-rays, Electronic discharge notes, Tablets to take home (this list is not exhaustive).

Instigate and co-ordinate daily meetings with ward discharge co-ordinators/ nurse in charge to understand and act upon required treatment plan, escalations. To attend all routine and emergency bed meetings to be given and feedback on escalations. To liaise with Head of Nursing and Matrons daily at the safety brief and represent the surgical division at bed meetings and provide required updates.

Be the divisional lead for ensuring that all CERNER discharge information is correct and updated so that all discharge audits are correct.

Be confident at working across divisions and directorates and have a proven track record at being able to undertake and lead challenging conversations with other members of the multidisciplinary team.
