Job summary

Working with colleagues across the three hospital sites of the Trust and in the Brent and Harrow communities, this post is focused on the implementation, delivery and development of a cardiac rehabilitation service (CRS). The aim of this post is to improve the care of patients with heart disease admitted to London Northwest Healthcare NHS Trust and after discharge to the Brent and Harrow communities.

The post holder will work with the CRS nurse lead enabling:

  • Identification and recruitment of appropriate patients into the CRS
  • Counselling and education
  • Designing personalised cardiac rehabilitation programmes for patients
  • Follow up and support after discharge using the Heart Manual and other local resources
  • Liaising and working with community colleagues

You will work autonomously assessing and managing patients in your caseload ensuring this reflects the core components of the Clinical Nurse Specialist role, which involves high levels of clinical decision making and advanced clinical practice. With training you will be expected to provide detailed advice to other specialties, advising the multidisciplinary team on the appropriate management of patient undergoing cardiac rehabilitation.

MAIN DUTIES

Clinical

  • Perform comprehensive assessment of patients nursing needs putting quality at the heart of practice by delivering evidence based individualized and personalised care
  • Demonstrate advanced nursing and clinical assessment skills for cardiac patients
  • Demonstrate advanced nursing skills in initiating cardiac rehabilitation pathways and determining appropriate referral.
  • Monitor investigation results, initiating further assessment/review as required.
  • Collect, collate, evaluate and report information, maintaining accurate patient records.
  • Act as a key worker for patients undergoing cardiac rehabilitation, working to coordinate care and provide accessible support for patients and carers.
  • Provide specialist advice at MDT meetings
  • Use advanced communication skills to identify and meet patients and carers needs.
  • Ensure systems are in place to ensure that the standards of nursing care delivered are safe, effective and compassionate to every patient, every time.
  • Actively work with other health care professionals to provide a seamless and coordinated patient care across primary, secondary and tertiary care interface to maximise patient satisfaction and outcomes.

Managerial / Leadership

  • Provide specialist, expert clinical knowledge to clinical colleagues, patients and carers/relatives following appropriate training
  • To work autonomously in planning and organising own workload in coordination with the cardiac rehabilitation nurse lead and other colleagues
  • Participate in the collection, collation and reporting of activity data to the team lead.
  • Deputise in the absence of senior cardiac rehabilitation nurse lead
  • Participate in the development and updating of protocols, operational policy and work plan
  • Participate in ensuring that national standards and core components for cardiovascular disease prevention and rehabilitation are adhered to through the collaboration with the cardiology team and the community services

  • Act as a role model/ preceptor and mentor to others, demonstrating high standards of care and providing clinical leadership and education regarding the care and support of patients affected by heart disease

Service Improvement

  • Continually evaluate the changing needs of the role and contribute in making recommendations for ongoing development of the service.
