Job summary

The post holder is a key member of the End of Life and Transformation Teams at LNWUH to lead on and embed the principles of Continuous Improvement, high quality care, productivity and efficiency into practice across all LNWUH sites through their leadership and expertise. The post holder will actively seek opportunities for innovative improvement in the delivery of end-of-life training and education and work with key stakeholders within the specialities across all sites to promote the uptake and delivery of this project.

Main duties of the job

The role will involve leading and supporting a team in the delivery of a transformation and improvement project for end-of-life training, supporting the realisation of significant strategic objectives. Also leading on a training programme for end of life care to be disseminated and sustained throughout the Trust.

  1. Walk the end of life educational pathways and map the processes, develop improvement plans (PDSA approach) and oversee their delivery
  2. Planning and organisation of end of life training methodologies, formulating and making recommendations as to the adjustment of plans or strategies on behalf of the end-of-life team

  • oEvidence of extensive end of life experience
  • oEvidence of building effective working relationships with clinical, operational, and multi-disciplinary teams
  • oProject management experience
  • oIn depth knowledge of the national and local health context
  • oExperience of managing teams, including objective setting, learning and development, performance management and allocation of workload
  • oSome knowledge of organisational development or management or leadership development theory, principles or programmes
  • oUnderstanding of current issues and challenges within health care delivery
  • oUnderstanding of quality improvement/safety improvement/process improvement/ clinical governance principles and their application
