Job summary

We're inviting applications for Substantive Consultant post specialising in General Haematology (we encourage applicants to develop an area of their own interest). This is an exciting opportunity to join an established and vibrant department in the expansion and development of clinical and diagnostic Haematology services within the North West London sector.

Based primarily at Northwick Park Hospital (NPH), Haematology services are delivered at all three Trust sites. Successful candidates will be part of a team of 16 Consultant Haematologist colleagues in delivering clinical, laboratory, diagnostic, administrative, research and teaching responsibilities.

In addition to a wide practice in General Haematology, the Department has a particularly active Haemato-Oncology commitment providing Level 3 inpatient care at NPH, a stand-alone MDT and a large NIHR-supported Clinical Trials Portfolio of international studies.

The Department is supported by 7 Specialist Trainees, 3 Senior Clinical Fellows, 3 SHO, 1 FYI and 10 Clinical Nurse Specialists, 3 Physician's Associates and is split into 2 clinical teams, Haemato-Oncology and General Haematology.

If you're successful there will be excellent opportunities to further develop clinical services, cultivate research ideas and to enhance teaching programs (at both post-graduate and undergraduate levels, as a major Centre for training students and trainees from Imperial College London).

Main duties of the job

The post is a new position, full-time 10 PA, in the Haematology Department of London North West Healthcare NHS Trust. The post-holder will be an integral part of the Haematology service provided to the Trust and the appointee will provide care for inpatient and outpatient haematology patients.

There are weekly myeloid, lymphoma, CLL and myeloma clinics within the department in addition to new referrals, general haematology, haemaglobinopathy and thrombosis clinics. The combination of clinics for the individual post-holder will be subject to their experience and interests. Full-time consultants at this trust will generally undertake 3 clinics per week.

There are weekly regional and local MDTs which will be part of job planned activity. Acute inpatient admissions are managed at NPH. For malignant haematology, Northwick Park has a stand-alone MDT and a weekly meeting with Hammersmith Hospital to discuss complex cases and transplant referrals. There is an opportunity for the post-holder to undertake a clinic at CMH site, but the post holder will predominantly be based at Northwick Park Hospital.

Within the department, there is good opportunity to significantly contribute to service development, particularly in the Thrombosis, Laboratory, ITP and Haemoglobinopathy services again dependent on experience and interest.

On-call attending occurs on a 1 in 6 basis

It is expected that the appointee's job plan will incorporate direct clinical care and supporting activity in a ratio of 8:2

Job responsibilities

Activity

The department is the busiest Level 3 Haematology centre in West London with the following activities:

  • Inpatients: There are inpatient beds now only at the NPH site consolidating the service at the Acute Trust. At NPH, there is a dedicated Haemato-oncology ward providing level 3 facilities with 15 designated isolation rooms and 7 bedded bay on Drake Ward and designated Haemoglobinopathy inpatient service on James Ward.
  • Outpatients: Clinics are held at all sites with subspecialisation for tumour sites, general haematology, coagulation disorders and sickle cell disease.
  • Day care: Day care unit with facilities for consultations, procedures, infusions and blood transfusions are performed at Northwick Park and Central Middlesex.
  • Anticoagulant service: All sites operate a hospital and community based service run by specialist nurses using the DAWN anticoagulant management support software.
  • Blood Transfusion: There is an active Hospital Transfusion Committee operating across and encompassing all sites of the Trust which participates in the SHOT enquiry. There is a Specialist Practitioner of Transfusion (SPOT) based at NPH and Trust-wide blood transfusion policy and guidelines are in place.
  • Malignant Haematology: The Department manages a wide range of disorders with the exception of high dose therapy with stem cell rescue and Stem Cell Transplantation. As part of the Royal Marsden Partners Vanguard (previously, the London Cancer Alliance), agreed network guidelines are followed with appropriate patients transferred out for Stem Cell Transplantation. To conform with IOG measures for cancer care, all Level 3 treatments for acute leukaemia and other inpatient Haemato-Oncology treatment (e.g. salvage for Lymphoma) is centralised in the Haematology Unit at NPH. All non-oral Level 1 and 2 chemotherapy is also delivered at NPH. The Department hosts an independent MDT based at NPH that caters for a population of more than one million, with transplant input from Imperial College Healthcare NHS Trust and University College London Hospital NHS Trust as well as specialist integrated Haematology malignancy diagnostic service input from Royal Marsden Hospital. Patients are entered into multi-centre clinical trials where possible and are discussed at the weekly MDT meetings at NPH (incorporating Histopathologist and Radiologist). There are close working relationships with Oncologists based at Mount Vernon Hospital (sessions held at NPH with radiotherapy at MVH) and with Orthopaedic surgeons at Royal National Orthopaedic Hospital in Stanmore with specialist interest in spine tumours (myeloma/lymphoma). A separate Haem-Spine MDT is held for patients under joint management. Each Consultant has a subspecialty interest in one of Lymphoma, Myeloma, CLL, Acute Leukaemia and Myeloid Neoplasms.
  • Outpatient chemotherapy care is provided at the dedicated chemotherapy suite at NPH delivering solid tumour as well as Haematological Cancer care.
  • Haemoglobinopathy (Sickle Cell and Thalassaemia): NHS England have commissioned the providers of specialised Haemoglobinopathy services, Haemoglobinopathy coordination centres for both Sickle Cell Disease and Thalassemia, and the National Haemoglobinopathy Panel following a review of Haemoglobinopathy services. LNWUHT is a designated specialist Haemoglobinopathy team site and Haemoglobinopathy coordinating centre for sickle cell disease. Outpatient and Day case work is situated at CMH. All acutely unwell patients with Haemoglobinopathy are hospitalised at NPH. Apheresis both elective and emergency is provided by NH Blood Transfusion service.
  • The Regional Neonatal Haemoglobinopathy screening and district antenatal screening service operate from CMH laboratories and counselling services are provided by the Brent Sickle Cell and Thalassaemia service.
