Job summary

This is an exciting opportunity to lead the older peoples and dementia service at the Trust. You will be working as part of the safeguarding team providing clinical leadership across the organisation.

Main duties of the job

  • To develop and improve access for older people throughout the Trust.
  • Alongside other professions in the Dementia Working Group, to develop a care pathway and a sustainable, robust dementia service for people with dementia admitted to London North West Healthcare Trust which includes Northwick Park Hospital, Central Middlesex and Ealing Hospital.
  • To ensure that patients and their families are fully informed and supported by providing clinical and social support where appropriate, and to be easily accessible to respond to family, clinical teams, patients and work flexibly to make a demonstrable impact in the delivery of dementia care.
  • To be a positive role model leader and an effective team player.
  • Influence strategic delivery of services ensuring quality of care and service improvement.
  • To provide clinical leadership, with a highly visible, authoritative presence and responsible for ensuring delivery of high quality care for people with dementia and their carer's.
  • To ensure that the needs of the person with dementia are placed at the centre of care delivery
  • To provide specialist, expert nursing advice to facilitate and support high-quality individualised care for people with dementia and their carers in collaboration with the multi-professional team and in line with local and national targets
