Job summary
We have a fantastic opportunity for a Therapy Care Assistant - Fixed Term 12 months.
The post holder will support patients and other staff to provide opportunities to be physically active to improve patient outcomes and act as the link between nursing, therapy and patients and their families on the ward.
Main duties of the job
- On Gladstone 2 and Gladstone 4 wards you will be required to be a team player
- To act as the key point of contact for activity and exercise for all patients on your ward, liaising with patients, family members, nursing staff, therapy staff and to be a key member of the ward team
- To assist the qualified nursing staff in providing physical support for patients, including assisting with transferring out of bed, walking on the ward and encouraging participation in exercise and activities on the ward. When necessary support with toileting and personal care.
- To assist in assessing patient's physical and functional needs in liaison with the therapy and qualified nursing team, including following care plans and exercise programs according and referring to the therapist where appropriate to discuss reviewing programs where necessary
- To assist in the maintenance of accurate records of exercise and mobility participation, including documentation in the medical records and exercise or activity diaries
- To assess a person's appropriateness to participate in exercise or activity, prior to commencing any tasks
- To monitor progress and participation in an activity, and reporting back any adverse effects during the session

- To work under the supervision of a qualified nurse and assist in providing exercise and activity support for patients
- To assist with transfers, mobility, seating, exercise and activity on the ward.
- To keep accurate records in relation to patient documentation and research data, including medical records, exercise diaries, mobility charts and nursing notes
- To ensure that a person is well enough to participate in any exercise or activity programme and feed back any adverse effects of the activity, escalating as appropriate if required wording for emergency response!!
- Works within guidelines of work schedule, however has freedom to implement improvements
- Supervise junior health care assistants making a contribution to their training and development
- To maintain accountability and responsibility

- To assist the qualified nursing and therapy staff in providing activity and mobility support to patients and implement creative ways of encouraging activity on the ward

- To have understanding and knowledge of using the equipment safely
- Escalate urgently any need for immediate medical support during or after any activity session using Trust escalation process
- Build relationships with family and carers in order to support the patients needs
- To promote independence, mobility and activity within the ward setting
- Recognise and respond appropriately to urgent and emergency situations
