Job summary

  • The Post holder will work in close liaison with the Matron, Clinical Nurse Manager and Ward nurses to facilitate the timely, effective and appropriate discharge of patients from the acute setting to the community, liaising with hospital, community, social and health service colleagues in order to promote a recovery focused discharge/transfer
  • Close liaison with patients, families, complex discharge team, social workers and therapists will be essential to ensure prompt but safe discharges of patients from our hospital setting

Main duties of the job

  • The post holder will also play a key role in the development of an innovative service providing excellent discharge practices within the ward environment.
  • The post holder will ensure best practice in planning safe and timely patient discharges in order to achieve best outcomes for patients as well as to optimise hospital bed utilisation and reduce length of stay.
  • The post holder will be expected to become expert in the homecare, supported accommodation and placement options within the local area, and be able to offer advice to staff on the process required to access them.
  • In conjunction with the Bed Manager, the post holder will also assist the multi-professional team giving support and advice and acting as a resource for all aspects of complex discharge planning.
  • The post holder will develop close and effective partnership working with colleagues in statutory health and social services, as well as voluntary sector provider
  • Clear understanding of safeguarding : its role and application in relation to patient discharge

Job responsibilities

The post holder will have excellent communication and organisational skills and be able to work under pressure and prioritise workload. They must be able to stay polite, patient, and courteous at all times to staff, patients and families,

The post holder will support the nursing teams in the flow of patients from the ward to the most appropriate discharge destination.

They will assess all patients social situation on admission and liaise with their families, care homes and social workers to obtain accurate information and plan safe discharge to appropriate destinations.

The post holder will maintain accurate discharge records either on paper or using electronic patient record system.

They will maintain key relationships and liaise with key members of the multidisciplinary team - Nurses, doctors, therapists, Complex Discharge Team, STARRS, Allied Health Professionals, social services, patients,next of kin/carers and Primary Care service providers.

They will be IT competent and able to utilise and update the various IT systems available in the Trust to support smooth patient discharge

They may have to cover and support for other discharge practitioners in the division and assist with some essential ward clerk administrative duties.

The post holder will have to assist in preparing and taking patients to the discharge lounge or to transport vehicles.

Keep up to date with mandatory training and have clear understanding of safeguarding procedures and policies
