An exciting opportunity has arisen for an dynamic, enthusiastic and highly motivated individual to join the Chemotherapy Day unit at Northwick Park Hospital as a Patient Navigator for the SACT Service
- The post holder is an integral part of the chemotherapy nursing team and is responsible for coordinating the care of chemotherapy patients through the chemotherapy treatment pathway
- The post holder will work under the direct or indirect supervision of the Senior Charge Nurse. They will be responsible for coordinating appointments, prescription of medication, appointment scheduling providing a point of access to patients and carers.
- This role has been specially designed to support existing pathways and will require the post holder to evolve to support the pathways as they are continuously improved over time. The post holder must be adaptable and willing to work flexibly as the service develops, which includes quality assurance, service development and improving patient outcomes and experience.
- The post holder will work both directly and indirectly with patients and their carers to assist in coordinating the care through from referral to treatment of cancer. The development of the role will be led by the needs of the patient. The role will also include providing the patient with the relevant information and coordinating any tests and appointments that are required to expedite the care pathway

- KEY RESPONSIBILITIES
- Ensure all patients on a SACT are scheduled to commence treatment within service KPIs / Cancer Waiting Times Standards as appropriate
- Proactively manage patient case load to aid the early detection of delays within the pathway
escalating any bottle necks to the Lead Chemotherapy Nurse.
- This post will evolve over time as the service grows and the post holder will be required to adapt and undertake any other duties as may be required by the service that are consistent with the responsibilities of the grade
- To comply with the Trusts and departments policies, guidelines, and procedures.
- Always maintain complete confidentiality. During these duties, the post holder will have access to confidential and sensitive material and on no account must this information be divulged to anyone other than authorised persons as directed by the Senior Nursing Team.
