Job summary

The Short-Term Assessment, Rehabilitation and Reablement Service (STARRS) is expanding, and we are looking for dynamic and motivated Band 6 Nurse to join our multidisciplinary team.

STARRS is an innovative service working with patients in the Harrow, Brent Ealing communities. STARRS incorporates a Single Point of Access (SPA), Nurses, Physiotherapists, Occupational therapists, and Therapy Technician staff.

As a Band 6 Physiotherapist you will be working in the Early Support Discharge within STARRS. You will have the opportunity to:

  • Gain experience in generic, orthopaedic, musculoskeletal, respiratory, and neurological rehabilitation, and falls prevention.
  • Take on extended roles such as venepuncture, drug administration, advance assessment of the acutely ill patient and acute injury assessment.
  • Supervise junior staff and students
  • Attend in-service training and have Continued Professional Development opportunities.

STARRS is committed to providing a 7-day therapy service and this role will include participation in weekends, Bank Holidays and late shifts.

Main duties of the job

Provide the highest standard of evidence-based nursing care to patients within their own home, working with agreed management care plans and protocols.

The post holder will provide assessment and treatment for patients with a wide range of complex physical, sensory, perceptual, and cognitive problems, including psychological clinical risk within A&E settings. Assessments may also include some inpatient assessments.

Optimise hospital bed utilisation and reduce the length of stay by ensuring the timely, safe transfer/discharges of patients to the most appropriate and agreed environment as well as preventing inappropriate hospital admission in line with the philosophy of the acute Trust and Clinical Commissioning Group (CCG).

Some elements of the service such as A&E, operate 08:00 to 22:30 hrs. Shift patterns will need to reflect these hours to provide continuity of the service. This vacancy shift cover is from 10:00 to 22:30 hrs.

Undertake the learning of new specialist skills and knowledge to include some physiotherapy and occupational therapy skills, assessing and planning social care packages in agreement with and on behalf of social services. All these skills are competency-led.

Develop specialist knowledge and skills in the management of long-term conditions, working with agreed Trust protocols in collaboration with clinicians and chronic disease practitioners
