Diabetes Specialist Midwife

Job summary

Working as the specialist midwife with enhanced education and training in diabetes,he/she will be best placed to provide additional support and guidance to a womenand her family, when her care becomes increasingly complex as a result ofdiabetes. The specialist midwife will evolve the diabetic care pathway and will workin collaboration with the diabetic nurse specialist and obstetric consultants whospecialise in diabetes.In this role the specialist midwife will act as an advisor to other midwives and willdemonstrate an advanced level of clinical decision making and responsibility. Theywill provide a continuum for the diabetic population which will enhance the health ofthe women and their families. The specialist midwife will continue to support, adviseand monitor women's health and wellbeing during her pregnancy and birth. He/shewill guide the women and her family on the care of the neonate and monitor itswellbeing. The specialist midwife will build up networks within the community towhich they will be able to sign post the women, enabling then to take responsibilityfor their future care.He/she will work closely with midwifery and obstetric colleagues to ensure that careprovision for the patients in the care pathways is of high quality, clinically effectiveand resource efficient.

Main duties of the job

Clinical? Undertake the full role of the midwife including advanced decision making, using evidence based practice and highly developed advanced practice skills.? Facilitate and have continuing responsibility for the effective assessment, implementation and evaluation of all midwifery care for women with diabetes within own sphere of clinical practice.? Develop and adhere to the maternity unit's philosophy and models of care to ensure that all policies and procedures are implemented and to lead on the development of evidenced based woman centered midwifery practice, both in the hospital and community.? Ensure that management plans of care incorporate all phases of the pathway: antenatal, intrapartum and postnatal.? Directly refer maternity users for medical investigations and for review to other health care professionals, as agreed by the lead consultant obstetrician.? Act as a resource for staff and provide support in relation to women using the service who have high risk pregnancies as a result of complex medical conditions.? Participate in the training and education of staff in relation to high risk pregnancy and complex medical conditions.? Provide a link between midwives, diabetes specialist nurse, general practitioners, health visitors, obstetricians and other agencies in relation to women's care.
