Job summary
1 The post holder will work within Southwark CAMHS community services to support effective patient flow, reduce delays in care transitions, and ensure timely access to appropriate services. This role integrates clinical expertise with system navigation and coordination. The practitioner will work collaboratively with multidisciplinary teams, families, and external agencies to facilitate care planning and discharge processes, ensuring continuity and quality of care for children and young people with mental health needs.
2. The post holder will work in partnership with other members of the CAMHS team, other CAMHS services, Child Health/Paediatric Services, and Children's Social Care and other partner agencies to support integrated practice and service delivery.
3. To provide specialist assessment and treatment to children, adolescents and their families referred to Southwark CAMHS.
4. The post holder will use specialist clinical knowledge and knowledge of child and adolescent mental health theories to carry out evidence-based assessments and interventions.
5. The post holder will manage their own small caseload and provide specialist therapeutic interventions to include work with parent and child together, individual work, parent work and family work. Also, direct work with children (e.g. CBT, psychotherapy/art therapy/play therapy) with both short term and long term interventions.
Main duties of the job
1. Lead on the coordination of patient flow across the Southwark CAMHS community pathways, ensuring timely access and discharge planning.
2. Work collaboratively with multidisciplinary teams to identify barriers to discharge and implement solutions.
3. Support and advice teams on recording care pathways effectively on epjs and monitor recording reports.
4. Monitor caseloads, length of stay, last contacts and service capacity to support effective resource allocation and reduce waiting times.
5. Liaise with external agencies and services to facilitate smooth transitions and continuity of care.
6. Provide clinical oversight and support to practitioners managing complex discharge cases.
7. With senior colleagues, evaluate and review the therapeutic strategic outcomes for the team and set aims and objectives and the monitor practice to assure quality standards of care.
8. Establish and maintain evidence-based practice in the assessment and treatment of young people in crisis or needing intensive and/or crisis intervention.
