Specialty Doctor - Rehabilitation Community Team

Job summary

The Southwark Assertive Outreach and Rehabilitation Team (AORT) supports recovery based outcomes for the most unwell patients in the borough, and has been well resourced with medical, psychological, OT and senior nursing staff in order to provide excellent care.

The team consists of three sub-teams, two of which provide mental healthcare for individuals in Southwark residential placements. The third sub-team supports patients living in independent accommodation who have high needs, are difficult to engage, or have frequent admissions to hospital.

A key aim of the service is to prevent admission to hospital as well as to promote recovery in those with severe and enduring mental illness.The service provides therapeutic interventions based on the model of recovery and social inclusion.

There are no exclusion criteria for the service. Inclusion criteria include:- A severe and enduring mental disorder with significant impact on functioning- A history of being admitted to hospital/HTT frequently or for long periods- Problems engaging with mental health services- Multiple and complex needs including several of the following:history of violence or persistent offending not meeting threshold for forensic services; significant and persistent risk of self harm or self neglect; treatment resistance; substance misuse; detention under the Mental Health Act (1983); unstable accommodation or homelessness.

Main duties of the job

1. To provide full time medical input to the service and assessment of patients in their own accommodation as required2. To undertake liaison with professionals outside the team including community and inpatient mental health services, GPs, housing providers, colleagues in the Acute Trust, and Adult Social Care, amongst others4. The doctor would assess under the Mental Health Act for admission where appropriate5. The doctor is responsible for record keeping including documentation of assessment and communication/ liaison with other agencies including primary care.6. The doctor will support the wider staff team. There will be opportunities to contribute to clinical governance, and formal and informal teaching.7. The doctor would be expected to be available to work 10 clinical sessions on the unit. The doctor will be able to take regular time for professional development as agreed with the team Consultant Psychiatrist.8. There will be job planning review within 3 months of appointment involving the team Consultant Psychiatrist and the team leader of the unit.9. The doctor would receive clinical supervision from the AORT Consultant Psychiatrist each week.
