Job summary
To work as part of the MDT for Home Treatment team providing daily crisis assessment, treatment and home visits to service users under the team. Provide an occupational therapy service primarily to the Home Treatment Team, working with a caseload of clients with complex needs, using evidence based/client centered principles to assess, plan, implement and evaluate occupational therapy interventions. You will provide occupational therapy leadership for students and/or junior staff, through supervision and appraisal. You will participate in the planning, development and evaluation of occupational therapy and multi-disciplinary team (MDT) services, holding responsibility for defined projects. To regularly supervise occupational therapy students on practice placements. To carry out MDT activities e.g. generic assessments and duty responsibilities as required.
*** THIS IS A FULL TIME, 4 DAYS A WEEK (8-6p.m.) ROLE. DUTIES WILL BE SPLIT EQUALLY BETWEEN HTT PRACTITIONER AND OT SPECIFIC DAYS.
THERE IS A REQUIREMENT OF WORKING A MINIMUM OF 1 WEEKEND A MONTH.
Main duties of the job

o To work with service users to identify occupational therapy goals as part of the overall care plan working within the care programme approach process.
o To plan and implement individual and/or group interventions, in collaboration with the service user (and their family), using graded activity to achieve therapeutic goals.
o To monitor, evaluate and modify treatment for service users with multiple needs in order to measure progress and ensure effectiveness of intervention.
o To demonstrate and apply a broad / high level of understanding of the effect of disability and recommend adaptations to the client's physical and social environment.
o To assess occupational needs of a defined client group.
o To undertake risk assessments and risk management plans.
o Provide clinical training and supervision to service users extended support network as appropriate.
