Band 8a- Clinical Service Lead
An excellent opportunity has arisen to work within the Southwark Right Care Team as the Clinical Service Lead.The post holder will be responsible for driving clinical and operational improvement in Right Care within the Southwark Directorate with day-to-day operational responsibility for understanding and driving down delays experienced by our service users accessing crisis and inpatient services.
The Clinical Service Lead will report to the General Manager for Crisis Services where they can escalate delays in resolving barriers to discharges from our inpatient wards. There is also an exciting opportunity to support the General Manager for Crisis Services in the development of innovative projects to improve crisis service offers and embed the Right Care Programme across the crisis and inpatient pathways.

Main duties of the job
The post holder will work closely with the community teams and inpatient wards to develop systems to unblock delays in discharge.The post holder will focus on unblocking barriers related to interface with community teams, accommodation, placement, welfare, domestic issues, advisory/funding panels, safeguarding issues etc.

The role reports directly to the inpatient and Crisis Services General Manager with strong links maintained with the Right Care Team, inpatient wards, Home Treatment Team, Psychiatric Liaison, ARC and the Trust Right Care Programme.
The purpose of the role is to ensure the directorate delivers a reduction in readmission, length of stay and bed occupancy on the inpatient units and more generally improve patients access, experience, and outcome.
The post will require excellent and highly developed communication, negotiation, and problem-solving skills.
The post holder will utilise highly specialist knowledge and expertise through comprehensive support in the assessment, complex planning, implementation, and overall evaluation of individual patient where a likely delay in discharge is indicated. In these circumstances, liaison with multi-disciplinary teams and external partners to plan complex discharges and resolve organisational or external issues impacting on a timely transfer of care are required.
The role necessitates strong and confident clinical decision-making ability.
The post-holder will understand the principles of critical thinking and positive risk taking and be confident to apply these in practice to aid staff in the management of Right Care.
The post holder will actively pursue actions relating to patients who are delayed discharges due to barriers. This will require the post holder to work collaboratively with all relevant parties including external agencies.
The post holder will be accountable for escalating internal and external issues and delays that cannot be resolved in a timely manner and provide assurance once a resolution is achieved. They are responsible for the accurate identification and reporting of delayed transfers of care (DTOC).
To form effective relationships with inpatient services to facilitate early effective discharge. Maintain and develop Primary Care liaison with an attached GP Practice as required.
To be responsible for facilitating student nurse training placements including offering teaching sessions and training. To undertake link working roles as delegated e.g. with Nursing Colleges, Carer Groups. To participate in project work with the team as delegated.
To abide by professional Code of Conduct and maintain current professional registration and revalidation. To abide by the policies and protocols of the South London and Maudsley NHS Trust and Local Authority.
To keep abreast of all developments pertaining to the provision of Right Care initiatives. To contribute to effective multidisciplinary team working.
To meet regularly with the General Manager for supervision, objective setting and appraisal.
To attend Team clinical review meetings and group supervision.
To keep the General Manager informed of any situation concerning service user or staff health and safety as necessary.
To input and update digital client information systems ensuring that entries are accurate and timely.
To participate in the management of change.
To undertake other duties appropriate to grade as requested by the General Manager.
