Mental Health Liaison Nurse - Diabetic Foot Service

Job summary

The aim of this post will be to enhance the mental health provision at King's College Hospital to the Multi-Disciplinary Diabetic Foot Team (MDFT) Network within the South East London (SEL) Integrated Care Board (ICB).

The SEL ICB has a network of 6 MDFT with 2 hubs, based at King's College Hospital (KCH) and St Thomas' Hospital, and 4 spoke hubs at Lewisham, Bromley, Greenwich and Bexley. The post holder will primarily be based at KCH and St Thomas' hospitals.

Main duties of the job

  • To work as a highly experienced mental health practitioner, carrying out specialist assessments and interventions as part of the MDFT.
  • Liaison between the foot care team and mental health team, initiating and facilitating interventions as part of the MDFT.
  • Ensuring all patients who are eligible receive timely and appropriate care, in the right place, at the right time, by the right person. An expansive knowledge of primary care, secondary mental health services, and third-sector organisations will therefore be paramount.
  • Enhance integrated working between MDFT and mental health teams to improve access to care, health outcomes and experience.
  • To coordinate patient care between MDFT and psychiatry inpatient and outpatient services, community mental health teams, GPs and family and community networks.
  • To develop a diabetic foot psychiatry education / training program for the MDFT and mental health professionals.
  • The knowledge of and ability to deliver evidence-based interventions in line with NICE guidelines will be integral to ensuring those who present in crisis are cared for holistically; the post-holder must also have a robust understanding of risk and be able to implement patient-focused risk management plans.
  • The post holder will function as a fully integrated member of the service's multidisciplinary team, supporting consistent achievement of the service's standards and aims, through their own individual practice and participation in quality improvement initiatives.
