Clinical Service Lead - Community Transformation
A permanent opportunity has arisen to support with the community transformation in Southwark.
The successful applicant will work closely with the other 8a Clinical Service leads across the community pathways. Your focus will be on supporting staff through the transformation changes and contributing to staff development. You will also support the development of the interface between the primary care mental health teams (PCMHT), long term community teams and other partner community teams such as Assertive Outreach Rehab, Early intervention and the Crisis teams.
This post is a great opportunity to develop or build on existing change management skills.

You will need to be innovative, dynamic and able to work autonomously.
Main Responsibilities
The Clinical Service Lead will support the implementation of transformation process. The role involves working across all the community teams in Southwark.
The post holder will lead on allocated tasks required for the implementation of the transformation. This will enable the community teams to continue to provide maximise effectiveness and efficiency of service provision throughout the ongoing transformation. The aim is to ensure integrated clinical, operational, quality and financial outcomes are achieved within the service line.

- To use and provide information required for the quality measure which will be collected & reported using dash boards to improve the quality of care delivery
