Job summary

  • The Senior Clinical Practitioner will support the clinical team leader in providing clinical leadership and managerial oversight of the community mental health rehab and recovery team. This includes supervision of care coordinators, leading on Quality Improvement in the team, taking lead in clinical decision making, and managing the referral pathway in an out of the CMHT.
  • The Senior Clinical Practitioner will contribute to the training and development of all junior staff within the team, through observations, modelling, co-facilitation, training, and ongoing support and ensure the staff within the team are equipped with the necessary knowledge, attitude and capabilities to operate effectively
  • To provide care co-ordination and case management to a defined caseload of service users within the framework of the Care Programme Approach (CPA). The ongoing caseload will consist of service users (18yrs-65yrs) who are experiencing severe and enduring mental illness. The interventions that are provided will be evidence-based and focus on engagement, recovery, rehabilitation and integration with the community, with the aim of upskilling individuals to reduce their dependence on supported living options.

Main duties of the job

Supporting budget reviews in terms of internal finance and supporting oversight of placements

Supporting delivery of evidence based rehabilitation interventions and strategies in the MDT

Teaching sessions around areas to support community recovery

Supporting day to day management of the team, including duty rosters, leave planning and supporting the team as a senior leader in the absence of the team leaders as a senior member of the teams leadership group.

Ensuring KPIS and audits are carried out as indicated and act on findings and implement planning strategies to address

Clinical Lead

3) To act as a clinical lead in the team, both informally when providing guidance to colleagues, and in Multi-Disciplinary Team meetings when crucial clinical decisions are being made.

a) To participate and contribute to team senior management meetings with Consultants, Lead Psychologists and Team Leader.

b) To attend meetings and forums, representing the team at various levels both within the Trust and with external agencies, ensuring that all staff are kept fully briefed on any new developments etc. The post-holder may be required to participate in specific interagency work relevant to the service.

Clinical Governance

Care Coordinating Responsibilities

Assessment: To formulate integrated needs assessments for service users requiring commissioned services and to monitor care packages as directed. To undertake risk assessments regularly, in liaison with all other health professionals and agencies involved. To ensure that risk assessments are regularly updated, are meaningful and are reflected in care plans.

Engagement: To assertively outreach difficult to engage service users. To meet service users regularly, to respond flexibly when agreeing venue, such as home visit, team base, education, work or recreational establishment to assist with engagement & encourage participation in treatment delivery as well as facilitating independence.

Support: To provide on-going care co-ordination within the CPA framework of a designated caseload of service users who are experiencing severe and enduring mental illness. To ensure that the Trust and Social Services CPA and Care Management protocols are followed and standards met. To provide support and advice in relation to service users regarding the issues concerning them. To work collaboratively with service users focussing on a recovery approach. Provide information to help service users make an informed choice about opportunities and services available to them and to then assist clients to obtain the required services, support and entitlement.

Interventions: To provide interventions relevant to patients under the Care Program Approach (CPA). To ensure all interventions are age appropriate and that care is sensitive to the needs of black and minority ethnic service users. To adopt a client centred approach embodied with the principles of the recovery model. To provide psychoeducation to services users. To co-facilitate, where agreed, therapeutic groups with a focus on service user rehabilitation reintegration and social inclusion.

Families and Carers: To involve families and carers in treatment plans where appropriate. This may require direct Family Intervention work. Ensuring services are provided within guidance of the Children & Families' Act and Care Act. To ensure that the needs of carers are assessed, and the care services are offered as necessary. To provide families and carers with psychoeducation.

Supervision and support:

The post-holder will line manage members of staff which will mainly consist of Care Coordinators. As an integrated services, the team supports both nursing and social workers within the generic care co-ordinator post. The post holder will ensure that each member of staff receive regular supervision and appraisal, and are offered essential and developmental training in accordance with their personal development plans and the needs of the service. The post holder will also follow the Trust policy as and when sickness or performance issues arise with supervisees.

a) The post holder will provide support and guidance to care coordinators, who are having difficulty with complex cases. This includes meeting clients with care coordinators.

b) The post holder will ensure that they have are able to prioritise their tasks so that they can support care coordinators to meet with high risk and/or complex clients.

Referral pathway:

To oversee the referral pathway within the team, this includes: helping to ensure that the team consistently meet the Waiting Time Standards, having oversight of referrals coming into the team, and helping to ensure that clients are discharged from the service per agreed care plan.

a) It is essential that the post holder develops a robust and competent understanding of the services eligibility criteria and supports clinicians in the service to develop their own understanding of the criteria.

b) The post holder will also assist the Team Leader in the allocation of cases to care coordinators.

Deputising:

To deputise on behalf of the Team Leader when the Team Leader is not present in the team. This includes undertaking tasks pertinent to the Team Leader role.
