Job summary
We are looking for a highly organised, resourceful individual with a genuine interest in least restrictive practice and enabling resolution of barriers that lead to people being delayed in restrictive hospital environments. The role of Complex Care Flow Lead is being offered as a 12 month secondment in the Complex Care Programme in the South London Partnership (SLP) which is a collaboration between three South London Mental Health Trusts.
The Complex Care Programme is responsible for access to circa 100 Inpatient Rehab beds hosted within the footprint of the three Trusts. The postholder would play a key role in overseeing movement in and out of these beds as well as supporting with discharges and step downs from spot purchase beds.
The successful candidate will be someone with experience of working with people with complex mental health issues with very strong communication skills and ability to liaise with a variety of stakeholders including health, social care, and others.

Main duties of the job
- To oversee the effective use of the c100 inpatient rehabilitation beds within the SLP footprint; namely Barefoot Lodge (provided by Oxleas), Westways, Heather Close, Tony Hillis Unit (provided by SLAM), Burntwood Villas and Phoenix Ward (provided by SWLSTGs).
- To ensure people remain in the least restrictive settings and optimum flow is achieved ie that people are transferred to rehabilitation in a timely manner, and that they are discharged when no longer requiring inpatient care.
- To offer support in assessment, complex planning, implementation and overall evaluation of individual service users where there is likely indicated delay in discharge. In these circumstances to liaise with MDTs and external partners to plan complex discharges and resolve organisational or external issues impacting on a timely transfer of care as required.
- To escalate internal and external factors that could delay discharge
- To collaborate with partners to recommend innovative solutions to enable effective post discharge support ie personal health budgets.
- With the Programme Analyst CCP Team, to develop a robust system of managing waiting lists to NHS and private providers, forecast discharges, and to have an overall knowledge of bed usage.
- To attend MADE (multi-agency discharge events) as required, across all MH Trusts.

About our locations:
The circa 100 inpatient rehab beds located within the 3 Trusts are as follows: Tony Hillis Unit (Lambeth Hospital SLAM moving to Ladywell Unit Lewisham Hospital SLAM), Burntwood Villas (Springfield Hospital SWLSTG) and Barefoot Lodge (Abbey Wood OXLEAS). The role will require you to support flow with patients on these wards but you will also be involved in supporting discharges from spot purchase beds. Travel to locations outside the 3 Trusts would be minimal.
The senior leadership team currently has offices at Lambeth Hospital but will be moving to Lewisham Town Hall upon Lambeth Hospital closure. The successful applicant has plenty of flexibility in terms of workplace when not required to be on the wards.

- Work in conjunction with the SLP Complex Care Single Point of Access (SPA) and the respective rehabilitation units and referrers to ensure referrals and admissions are managed in a timely manner.
- Undertake trusted assessments for people being considered for admission by the SPA
- Ensure that service users who are referred for admission, meet eligibility criteria prior to admission ie their circumstances/health have not changed so that they are no longer eligible
- Use clinical knowledge to oversee appropriate use of rehab beds as a system ensuring people are cared for in the least restrictive setting possible for the shortest possible time. At times it may be appropriate to move someone between rehabilitation wards however this should always be in the persons best interest and not extend their total length of stay within inpatient care.
- To work closely with rehabilitation ward teams to support MDTs to be proactive about discharge planning from day one of admission.
- Be actively involved in the planning and co-ordination of service users journey into appropriate placements
- Have awareness of the process relevant to placement panels to support situations where an aspect of the discharge pathway is delayed or unknown
- To have an understanding of pathways between services facilitating close working relationships where advantageous ie fostering step down opportunities to block housing provision commissioned by the MH Trust.
- Assist in the development and implementation of a strategic framework for service user flow through the complex care pathway.
- Have direct communication with inpatient and community teams to develop instrumental relationships that support the flow processes.
Management of information
- With the Programme analyst, maintain an accurate understanding of who is waiting for inpatient rehabilitation both within NHS and private rehabilitation settings.
- To identify who this should be communicated to so as to ensure a full understanding of system flow
- To have an overview of the trusted assessment process supporting the Consultant Psychiatrist in allocation of trusted assessments, - maintaining an accurate understanding of outcome ensuring reports are written up in time for dissemination. Currently there is a 7- day target from presentation to the SPA to outcome of the referral being known to the person who has referred the individual.
- To maintain accurate records of expected discharge dates, together with those who are delayed ensuring there is a joined-up action plan to facilitate discharge
- Provide oversight of people receiving care in the private sector and repatriate them to NHS beds when appropriate.
- Escalate issues that cannot be resolved to Senior Leadership Team (SLT) where appropriate maintain an action log until discharge is completed.
- Comply with Trust Bed Management Policy and its processes and escalations.
- Updating all relevant processes, forms and documentations on Clinical Systems.
- Have an overview of breach times and other relevant data to support with Clinical prioritisation.
Service improvement and innovation
- To assist in facilitating specific projects, including QI projects that may be relevant to the improvement of service user flow.
- Conduct internal audits for quality assurance.
- Work with the personal health budget team to support the commissioning of bespoke packages of care where needed.
- Raise to senior SLP management where there are genuine gaps in service provision to feed into commissioning plans.
- To produce reports on service performance and development as required
- To be a change agent driving a cycle of continuous quality improvement using lean methodology and other relevant techniques as required
