Job summary

We would like to invite applications for the exciting opportunity of Advanced Practitioner - Adult Community Eating Disorders Service for the Enhanced Treatment Team (ETT).

The main objective of the ETT is to offer an individualised package of care for patients, at home, for a limited time between either Outpatients, Inpatients or Day Care. Each patient would be held for approx. six weeks and receive intensive input during this period.

The Advanced Practitioner would play a key role in ensuring patients' needs are met, care plans are individualised according to need and liaising with other parts of the MDT pathway.

You will have strong organisational skills and have the necessary communication skills to ensure clear and timely liaison between the patient, GP and other parts of the Eating Disorders service, across the South London Partnership.

Main duties of the job

The post holder will work as an experienced health professional (Nurse, OT or Social Worker) in eating disorders, who acting within their own professional boundaries will provide care for presenting service users from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.

Advanced Practitioners co-ordinate multidisciplinary working, maintaining excellent communication and working relationships with all team members, and other services within the Trust, to ensure effective interface and continuity of care between disciplines and services. They are expected to pro-actively manage the caseload ensuring that the service maintains a robust throughput of patients.

The Eating Disorder Unit (EDU) is internationally renowned for providing best practice, evidence-based care for adults with the full range of eating disorder difficulties. Outpatient treatments include guided self-help, group and individual interventions. We are the developers of MANTRA, now recommended as part of NICE 2017 guidelines for the treatment of Anorexia Nervosa, and of FREED (First Episode Rapid Early Intervention for Eating Disorders).

6.1 Clinical

To ensure thorough risk assessment of all service users is line with policy standards

To ensure that the team assess, diagnose, plan, implement and evaluate effective interventions for service users presenting with mental health diagnoses and any co morbid conditions, and to be available to advise on the most complex and high risk cases

To ensure the team assesses capacity and provides safe treatment and care to those who may lack capacity.

Rapidly and continuously evaluates the patients changing condition and response to therapeutic interventions and modifies the plan of care for optimal patient outcome.

Attend high risk or crisis situations to provide management advice and clinical intervention

To operate within the legal requirements of the Mental Health Act and be able to exercise independent judgement in the assessment and diagnosis of mental health conditions.

To assist the team to effectively prioritise demands on its resources shift by shift, including rapid response to emergencies.

6.2 Communication

Communicates the patients health status using appropriate terminology, format, and technology

Act as an advocate for service users.

Establish effective communication and actively promote health education that may include providing alternative materials to support service users and carers in making decisions regarding their care and treatment

Liaise and communicate effectively with multidisciplinary and primary care teams across health and social care boundaries, ensuring management of throughput

Responsible for the safe and effective discharge of patients and smooth transition of care from the ETT to other services.

Ensure the team communicates effectively as a whole, including use of productive handovers, MDT meetings and team members use of the electronic patient record.

Maintain accurate and contemporaneous records of health assessment, implementation and evaluation of management and therapeutic interventions.

Ensure effective liaison with all parts of the service including daily communication with those responsible for gatekeeping, bed management and accurate data collection.

6.3 Leadership, Policy and service development

On a day to day basis acts as the go to senior person on the shift.

To professionally maintain accurate records and ensure that standards of record keeping are maintained by those supervised by the post holder.

Ensures that through the supervisory structure high standards are maintained for clinical activity including making sure robust risk assessments are undertaken and documented along with sound care planning.

To assist in the monitoring of safe practice and take a lead on fact--finding investigations in the team.

To take a lead on all Safeguarding issues within the team on a shift by shift basis

Provides the highest standard of professional and expert clinical leadership that influences advanced level of practice in mental health and demonstrates positive role modelling of this role within their profession.

Support staff development within their clinical and professional competencies.

Encourage other staff to self assess their knowledge and skills of mental health practice, and challenge actions that are not in the interest of service users or the public.

Evaluate and review with the Clinical Service Lead service provision and innovations, and take a lead on clinical audits across the service.
