Job summary

An exciting opportunity has arisen for a Care Coordinator, either a Nurse, Social worker or an Occupational health Therapist, to work within the Croydon Community Forensic Mental Health Team.

We are looking to find a dynamic, creative, forward thinking and hard-working individual to work within our friendly team.

The Post Holder will work as a member of a borough based Forensic Community Team and will manage a reasonable caseload of clients with offending history and mental health problems.

The post holder will aim to provide a flexible, responsive and pro-active programme of care and support for the service users and their carers. you will be required to develop and maintain links across a wide variety of statutory and non statutory organisations such as Probation, MAPPA, Jigsaw, Hostels and other voluntary and non voluntary services. You will also be involved in specialist mental health assessments and the application of evidence based psychosocial interventions.

Main duties of the job

As a Care coordinator you will work with primarily Forensic clients in Locked rehabilitation, Residential Care, medium support settings and community based service users in their own accommodation within Borough. Supervise and care co-ordinated service users discharged to supported community placements. Monitor, evaluate and develop best clinical practices for patients in community and medium support residential settings providing services for Croydonservice users.

Care will be provided in accordance with service user need and assessed risk. A tiered model of care allows flexibility in terms of service users being managed in the least restrictive setting, whilst empowering general mental health services to become collaboratively involved in the clinical management of this clinical group

Service users who are referred for forensic community follow up will have more than one multidisciplinary team member involved in their care and will be on CPA. An assertive approach will be practiced in their management whilst in the community.
