An exciting opportunity has arisen for a 12-month secondment position as a Specialist Mental Health Practitioner for the Mental Health Liaison for Older Adult team - GSTT. This is an ideal post for someone looking to develop and consolidate clinical leadership skills and develop management skills within a team of skilled professionals.
We are looking for an enthusiastic and motivated applicant who has the passion of working with older adults and wants to challenge themselves to develop their clinical and leadership skills in a fast paced environment. The successful applicants will have a proven ability to lead in implementing and sustaining changes, sound clinical skills and an ability to work collaboratively in bringing people together.
Main duties of the job
The post-holder will work closely with Older Adults and Working Age Liaison consultant psychiatrists, mental health liaison team practitioners, geriatricians, acute physicians and the frailty team at Guy's and St. Thomas' [GSTT] Hospitals:
- provide biopsychosocial assessment, risk and clinical management and collaborative, parallel working with medical colleagues for patients suffering psychiatric symptoms whilst under the in-patient care of GSTT
- work to expedite safe discharge, prevent inappropriate readmission, facilitate community support and contingencies and expedite appropriate, timely discharge from GSTT wards.
- hold a small caseload; providing structured intervention for patients seen.
- provide advice on the management of patients subject to the Mental Health and Mental Capacity Acts.
- work with the older adults' home treatment teams, older adults specialist care home intervention teams, relevant acute trusts community services, CMHTs, primary care, social services, and third sector agencies, etc.
- work alongside a small mental health older adult multidisciplinary team [MDT] whilst undertaking joint and autonomous assessments.
About us
Our Trust headquarters is located at Denmark Hill less than 5 minutes from the train station (zone 2). We also provide services and operate across other locations, such as London boroughs of Croydon, Lambeth, Lewisham and Southwark; and substance misuse services for residents of Bexley, Lambeth, Greenwich and Wandsworth.

Liaison and perinatal psychiatry services are provided by SLaM across King's College Hospital, Croydon University Hospital and University Hospital Lewisham, as well as at GSTT.
St Thomas' hospital is an 800 bedded flagship teaching hospital with a busy Emergency Department and multiple tertiary specialist services. Guy's hospital has 400 beds, extensive tertiary specialist services, and an Urgent Care Centre.

- RGN/physical healthcare qualification
Experience

- Previous or current experience of working with Older Adults
- Experience of undertaking assessments in all areas of acute hospital including the Emergency Department setting
- Experience of assessment and management of patients who have harmed themselves
- Experience of working with patients with both physical and mental health issues
- Experience in the management of psychiatric emergencies
- Experience of recording, managing and analysing data as part of audit, research, or service evaluation
- Experience of providing clinical supervision

- Experience of managing large volumes of data and providing analysis
