Band 4 - Nursing Associate, Belsize Priory
As a Nursing Associate working in a Community Camden Integrated South Neighbourhood Team, based at Hunter Street Health Centre. You will provide nursing interventions to a range of housebound patients and help to manage their care supported by a skilled team of nurses. We are proactive in looking for innovative nursing associates who will facilitate both the implementation and development of new ways of working and want to progress their career.
Main duties of the job
The team is supported by an overall Neighbourhood Lead in our Integrated Locality Team. We also have a range of Clinical Practice Educators and Professional Leads who are on hand for supervision and support. You will also work alongside Complex Care Nurses, Occupational Therapists and Physiotherapists, pharmacy technicians, students and a you will have a dedicated mentor.
As well as your integrated care colleagues, you will have access to and support from the wider expertise of our specialist teams including Evening and Overnight Nursing team, early supported discharge team, Rapid Response, Palliative Care, Tissue Viability, bladder & bowel, community stroke and neurology team, respiratory, heart failure and diabetes teams as well as the in-patient wards: who all aim to work very collaboratively.Your role will include working closely with our GP colleagues and practices.

As a Nurse Associate you are expected to:
- Understand and act in line with NMC professional standards for practice contained within the Standards of Proficiency for Nursing Associates.
- Manage a delegated active caseload, working as a member of a skill mixed team, responsible for interacting constructively with team members and organising, prioritising and planning their work.
- Use a range of assessment tools pertinent to the patients needs, to inform the assessment and assess risk for both patients and staff, including the recognition and assessment of patients at risk of abuse, self-harm/and or suicidal ideation and the situations that may put themselves and others at risk. Provide advice and support to patients to enable them to make informed choices in areas such as: diet, smoking and alcohol.
- Understand when to seek appropriate advice to manage a risk and avoid compromising quality of care and health outcomes acting in line with local and national organisational frameworks, legislation and regulations to report risks, and implement actions as instructed.
- Demonstrate the ability to effectively and responsibly access, input, and apply information and data using a range of methods including digital technologies, and share appropriately within interdisciplinary teams.
- Demonstrate an ability to prioritise and manage their own workload, and recognise were elements of care can safely be delegated to other colleagues, carers and family members
- Contribute to the provision of care for people, including those with complex needs.
Patient Care
- Provide sensitive, compassionate, safe and effective care to support people in a range of care settings, applying the principles and processes for making reasonable adjustments.
- Develop understanding of all elements of the nursing process and be able to assist the Registered Nurse in the on-going assessment, planning, management and evaluation of care.
- Perform and record clinical observations including blood pressure, temperature, respirations, and pulse.
- Undertake clinical tasks including catheterisation, venepuncture and ECGs. This is not an exhaustive list; additional competency based skills relevant to clinical area can be attained.
- Accurately record patients nutritional and fluid intake as per plan of care.
- Ensure the privacy, dignity and safety of individuals is maintained at all times.
- Demonstrate the ability to recognise changing priorities seeking advice and guidance from the Registered Nurse or other Allied Healthcare Professionals as appropriate.
- Ensure that high standards of nursing care are given and maintained. To act and raise concerns when standards are not being maintained following local policy and procedures.
- Administer medications within local policy and procedure and in line with the NMC Standards of Proficiency.
- Recognise changes in a patients condition which require the intervention of others and understand when to refer to the supervising Registered Nurse and other senior colleagues for reassessment of patient care.
