Job summary
Be part of an exciting new initiative in Brent - the Older Adult Urgent Care, Flow & Frailty Service - designed to transform care for older adults with complex mental health needs, frailty, and dementia. As a Band 7 OT Lead and Team Lead you will play a pivotal role delivering specialist input across settings. You will work closely with emergency departments, care homes, community teams, primary care, and voluntary sector partners to ensure timely, preventative, and person-centred support that makes a meaningful difference to patients and their families. The team's focus will be on responsive coordination, clinical decision-making, and proactive intervention to help individuals access the right care at the right time.
You will provide day-to-day leadership and oversight for the team's workload, as well as supervision to the 2 x Band 6 Mental Health Practitioners and guidance to Band 4 colleagues (Supervised by Band 6 practitioners). You will work collaboratively with carers, families, and partner organisations to enhance the patient experience.
Your work will be instrumental in reducing avoidable hospital admissions, improving system flow, and strengthening integrated, community-based support for older adults in Brent. Main duties of the job
The postholder will expert OT assessment, intervention, and care planning for older adults and people living with dementia and/or mental health across community, care home, and home settings.
The role supports safe discharge, continuity of care, and preventative interventions to reduce avoidable hospital admissions, integrating mental health within frailty management.
The postholder will participate in Integrated Neighbourhood Teams and multidisciplinary forums, providing specialist guidance to colleagues, supporting care homes and primary care, and leading evidence-based cognitive and social interventions.
They will utilise digital tools for monitoring and follow-up, maintain strong communication with internal and external partners, contribute to governance, audit, and service development, and maintain professional standards and registration to deliver high-quality, person-centred care.

- o Degree in Occupational Therapy (BSc/MSc)
- o Practice education qualification or willingness to obtain
- o Evidence of extensive post graduate CPD relevant to frailty, mental health, cognitive assessment, functional rehabilitation or urgent community care
- o HCPC registration (maintained and in good standing)
Desirable
- o Accredited training in specialist assessments (e.g., MoCA, ACE-III, functional cognition tools)
- o Quality Improvement (QI) training
- o Post graduate qualifications in frailty, dementia, mental health, leadership or advanced clinical practice
