Job summary
CNWL is looking for an enthusiastic and highly motivated Specialty Doctor to work for the Crisis Resolution and Home Treatment Team, based at Park Royal Hospital.
The post holder will have a central role in providing medical input within the well-established community mental health team. They will work closely with the other members of the multi-disciplinary team, in order to ensure that patients are provided with the highest standards of care. You will be supervised and supported by substantive Consultant within the Team.
Main duties of the job
o Provide clinical interventions for the patients managed by the team and support members of the multidisciplinary team.o Participate in daily morning MDT, CRM, taking minutes.o Medical review for patients under the care of HTT, in the clinic and home visitso Prescribe medication, review and optimise medicationo Physical health investigation -ordering ECG, blood test and filling blood test in S1 records
o Advising HTT nurses and AP regarding medication and medical reviewso Managing admin MH4 letters, GP discharge notification and any other admin related to medical inputo Working closely with the AMHP team and making referral/ first medical recommendation for MHAAo Provide good service quality and performanceo Support the training needs of psychiatry trainees, medical students and the MDT team
About us
Brent Mental Health Service has been operating as a multi-disciplinary, integrated service since 1st April 2001. The integrated service relates to Brent Adult Social Services department as well as to CNWL and currently includes all generic and specialist adult services such as forensic, inpatients, psychotherapy and residential services, including the following:
o Acute in-patients (4 ward - 70 beds) - Park Royal Centre for Mental Health.o Community Mental Health Teams (CMHT) - organised in three locality CMHT teams (Harness, Kilburn and Kingsbury & Willesden) serving the three primary care networks (PCNs) in the borough based at Park Royal and Fairfield House.
o Early Intervention Service based at 27-29 Fairlight Avenue, London/
o Forensic and Intensive Care Services which include Tasman Ward (Forensic Low Secure-17 beds), Caspian Ward (PICU - 13 beds) and Java (Forensic Rehabilitation - 6 beds) - Park Royal Centre for Mental Health.
o Home Treatment Team (HTT) - Park Royal Centre for Mental Health.o Mental Health Act Team (AMHP Team) - Park Royal Centre for Mental Health.o Psychology and Psychotherapy Services including Clinical Psychologists embedded in the CMHTs and inpatient unit, Arts Psychotherapy, and other specialist family and individual psychotherapies - Fairfield House.
o Range of rehabilitation services, supported tenancy schemes and residential care services located across the Borough

Clinical duties: 1. Assessment (including assessment of risk), treatment and management of adults with mental health problems (functional and organic). 2. Treatment, using a sound knowledge of the evidence for pharmacological and non-pharmacological interventions, in accordance with NICE and relevant guidance. 3. Formulating psychiatric opinions about a bio-psycho-social care plan. 4. Full participation in the multidisciplinary team providing care in the community. 5. Review and monitoring of their caseload to ensure appropriate care packages are put in place and discharges facilitated to maintain patient flow. 6. Domiciliary visits to provide assessment and treatment services and in response to emergencies. 7. Contribution to care quality through quality improvement (QI) projects and relevant participation in local and Trust wide care quality processes. 8. Active involvement in teaching of undergraduate medical students. 9. Supervising the clinical work of the trainee and multidisciplinary colleagues. 10. Psychiatric assessments of residents who may be in crisis in another borough. 11. Close liaison with the wards to facilitate admission and discharge planning. 12. Liaison with HTT, Psychiatric Liaison Services, AMHP services and Forensic Services in and outside the borough. 13. Provide support to staff in the different applications of Mental Health Act and Mental Capacity Act. 14. Supporting the MDT team to provide the best clinical care for patients. 15. Accurate record keeping according to the Trusts and professional guidelines. 16. Supporting the Consultant Psychiatrists in chairing MDT, CPA, professionals and best interest meetings and preparing medical reports for MHA Tribunals/Managers Hearing.
