Job summary
This is an exciting opportunity for any health care support worker who is looking for new challenges and opportunities to enhance their skills. The Rehabilitation Assistants work as part of the multidisciplinary team to assist in the provision of support, rehabilitation and re-enablement of patients who have been referred to the service following a hospital admission or for the avoidance of unnecessary hospital admissions.
Rehabilitation Assistants work under the supervision and guidance of qualified Occupational Therapists and Physiotherapists to provide rehabilitation in the patients' home environment with a focus on reducing care needs in the community. The Rehabilitation Assistants follow agreed protocols and evidence-based guidelines to deliver care which is safe and effective for the patients.
The service operates 5 days per week with core hours Monday to Friday between 8.30 am - 4.30 pm, with flexible working patterns available.
The post holder will need to be flexible to meet the needs of the Service.
Main duties of the job
To work as a Rehab Assistant within a multidisciplinary team, providing rehabilitation and/or supported discharge to people in their own home or place of residence and/or designated buildings.
You will work under the guidance of health care professionals to assist therapists by carrying out delegated aspects of patient care without direct supervision and reporting back patient's progress or issues to the therapist.
To be responsible for ordering equipment and department supplies
To undertake specific skilled support work to aid the rehabilitation of patients in the community.
