Band 8a Lead Nurse for Guided Care Matrons
To work in collaboration with partner organisations providing strong operational nurse leadership to the Guided Care Matrons and team members as part of the Care Connection Team (CCT) to ensure the delivery of high standards of patient centred nursing care for the residents of Hillingdon.
Main duties of the job
An opportunity has arisen to co-lead the Care Connection Teams (CCT) . Alongside one other lead, you will be responsible for the leadership, management and development of the Guided Care Matrons and otehr members of staff within the Care Connection Teams (CCT).
The Care Connection Teams are part of the Hillingdon Health Care Partnership (HHCP) . They proactively support people over 18 years old that have long term conditions and are at risk of hospital admission. The Care Connection Teams are aligned to neighbourhoods, based in GP practices. They are a team comprised of Guided Care matrons, GP's, Senior Administrator Coordinators, Health Care Support Workers, Pharmacists, Health and Well-being Officers and Mental Health Nurses.
