Are you passionate about making a difference in the lives of young adults navigating mental health challenges? A new exciting opportunity has arisen for a Band 7 Young Adults Pathway practitioner to join the Bentley House Community Health Hub.
We are looking for a qualified mental health clinician to work with the Harrow Young Adult Pathway Lead and provide a high quality, accessible mental health pathway for 16-25 year olds ("young adults") in the borough. The pathway aligns with the Trust's model of care for Young Adults and looks to provide a high quality, safe and effective pathway tailored for the 16-25 age range, aiming to reduce gaps, address unmet needs, facilitate smooth transitions, and ensure a positive service user experience.
You will need to be a dynamic and flexible team player with excellent communication skills. Previous work experience in supporting individuals with mental health and neurodiverse needs would be desirable. The service is a short walk from Harrow & Wealdstone station with good transport links.
Main duties of the job
To work with the Young Adults Pathway Lead to identify and support young people with mental illness and at risk mental state patients across the whole systems.
To focus on health inequalities for young people between 18-25 to deliver a holistic care to young people using bio-psycho-social approach which is trauma informed.
Maintaining and developing the good relationship between EIS Harrow, Harrow Whole systems and young adults partnership model of care team (Harrow YAP) in delivering a service to young adults at risk of serious mental illness.
The post holder will require excellent engagement skills, demonstrating an anti-oppressive and enabling approach to co-production with the young people patients. The person should be skilled in assessing mental health, creating risk assessments, safety plans using DIALOG Plus and crisis and contingency plans.

The Bentley House Community Mental Health Hub operate an intervention-based model and utilise a Trauma-informed approach to working with our service users. Regular supervision and access to opportunities for professional development will be offered to the successful postholder.
