Main duties of the job
- Oversee & undertake physical health checks & offer health promotion support to complex patients with severe & enduring mental health problems who are under the care of the low secure forensic services & community forensic team.
- Provide advice, supporting clinical & social care interventions for service users whilst holding a caseload of community forensic patients.
- Supervise & support the teams to deliver high standardsof physical care to patients with long term conditionsin accordance with up to date, evidence-based professional practice subject to local & Trust policies & to provide leadership to the nursing teams within the services. Focusing on supporting patients to be healthy, monitoring of long-term conditions, health promotion & screening.
- Lead on preparation & delivery of health promotion initiatives linking in with Trust resources, local business & healthcare providers. Ensuring that health promotion materials are up to date, available in various languages & easily accessible across the services.
- Support the administration and facilitation of service user forums & feedback relating to physical health and health promotion initiatives.
