Specialty Doctor in Victoria Community Mental Health Team, Westminster

Job summary

We are looking for a full-time Specialty Doctor in the heart of London and to work in Victoria Community Mental Health Team, an established multidisciplinary service in Westminster.

The post is an established (and hence replacement) post within the CMHT to support service delivery in line with Trust operations and clinical strategy. It has become available due to the current post holder leaving to pursue interest in a psychotherapy career.

Main duties of the job

The specialty doctor in post will form an integral part of the senior leadership team. The post holder would be encouraged to undertake an audit or quality improvement project and would have ring-fenced time for this.This is an 10 PA Specialty doctor post in Victoria PCN. Dr Stephen Ginn is the substantive consultant and named clinical supervisor for this post. He is also the Open dialogue Implementation Lead in the South hub. Dr Sara Ketteley (8 PAs with 6 PA in the team) is the other substantive consultant in the team and is also a Clinical tutor for trainees; she has trained in Open Dialogue during the last year. There is a higher trainee (ST4-6) and a junior trainee (CT1-3) in the team.

Westminster adult mental health service provides a range of mental health services for adults within the community and in hospital settings. Two mental health hubs provide an overarching structure in the North and South of the borough, within which there are functional groupings serving each PCN.

The team receives about 50 referrals/ month and has a caseload of about 400. The model of care is detailed in the Standard Operating Procedure but key operating principles are:1. No wrong door- diagnosis and severity are not obstacles to referral2. Intensive triage enables the right assessment and intervention early3. Focused and named intervention enables quick help and rapid throughput4. High level liaison and physical presence in GP surgeries enables pre-empting of referrals as well as early ending of interventions and support for patients in primary care

Job responsibilities

1. Conduct initial clinical assessments and medical reviews as allocated in team zoning discussions/by consultant supervisor.2. Provide consultation to the rest of the team, undertaking reviews with care co-ordinators, attending professionals meetings.3. Carry out home visits and patient reviews/assessments in care homes as indicated 4. Help embed open dialogue practice in the team once trained and where indicated/preferred by patient and staff5. Mental Health Act assessments also form a core part of the role. If the post holder is not section 12 approved he/she would be supported to become Sec 12 approved to carry out this function in the team.6. The post holder would be expected to hold a caseload and to maintain updated Excel spreadsheet at all times for supervision oversight purposes. He/she can access supervision support from the named clinical supervisor.7. Attend team clinical zoning meetings, team reflection meetings and service development meetings as needed.8. Carry out liaison and collaborative working with other services/agencies and across internal interfaces as necessary for patient care and service development.9. Ensure that patients physical health needs are addressed.10. Carry out regular review and monitoring of the caseload including via consultant supervision to ensure appropriate care packages are put in place and discharges facilitated to maintain patient flow.11. Ensure all documentation is completed in a timely manner including Dialogue plus, physical health recording, progress note entries, zoning discussions and risk assessments for patients on caseload.12. Keep up to date with appraisals, mandatory training, all revalidation requirements.13. Attend an SAS peer group for RCpsych CPD purposes and remain in good standing with RCPsych for CPD at all times.
